Physical Therapist

$15,000 Sign-On Bonus

Summary:

Provide professional physical therapy services in accordance with physician orders, state regulations, and organizational policies to support the rehabilitation and wellness of clients receiving home health care.

Supervision Provided To:

+ Physical Therapist Assistants (PTAs)

+ Home Health Aides (if permitted by State Practice Acts)

Essential Functions:

+ Collaborate with the Team Leader to prioritize and plan visits based on client needs and physician instructions.

+ Maintain current knowledge of laws, regulations, and internal policies.

+ Perform comprehensive client assessments and develop individualized treatment plans with realistic, achievable goals.

+ Coordinate care and make referrals to other disciplines as appropriate.

+ Monitor client condition and communicate changes to the physician; modify treatment plans accordingly.

+ Apply preventive, therapeutic, and rehabilitative techniques as appropriate.

+ Administer manual exercises and physical modalities, and instruct clients in independent exercises and daily living activities.

+ Evaluate, adjust, and educate clients on prosthetic and orthotic devices.

+ Instruct clients, families, and caregivers in therapy techniques to be continued independently.

+ Actively participate in interdisciplinary care conferences and quality improvement initiatives.

+ Complete all required documentation promptly and accurately.

Minimum Qualifications:

+ Degree from an accredited physical therapy program approved by the **American Physical Therapy Association (APTA)**

+ Active state license to practice as a Physical Therapist

+ Minimum **1 year of experience** , or completion of **6–8 weeks of preceptorship**

+ Knowledge of Medicare Conditions of Participation and home health care experience preferred

+ Self-motivated with strong organizational and time-management skills

+ Valid driver's license and access to reliable transportation

+ Commitment to upholding the **organization’s mission, ethics, and compliance standards**

Physical & Mental Requirements:

+ Ability to **lift, stand, and walk** for up to 50% of time

+ Frequent **bending, stooping, twisting** , and patient positioning

+ Lift up to **35 lbs** independently

+ Perform detailed tasks with frequent interruptions

+ Adaptability to changing schedules and work environments

+ Travel required to various **Trinity Health** or client locations
