    The Care Coordination Assistant Responsibilities Include:

    + Entering new referrals and updates all existing patient information in corporate IS systems including demographics, insurance, and benefit information.

    + Verifying insurance and benefits for all Medicare, Managed Care and Private Fee for Service Plans. Securing authorization for services provided by BayCare HomeCare from the payers.

    + Selecting the correct payer codes to provide accurate intake information.

    + Understanding medical criteria for securing all required information from the referral source.

    + Entering the appropriate financial information including benefits, deductibles, co-pays, coinsurance, out-of-pocket, another related costs into the EMR system.

    + Timely requesting and communicating of authorizations to payer and documentation in EMR.

    + The Care Coordination Assistant will work under the direction of the Care Transition Coordinator or RN Case Manager to establish and maintain a positive working relationship with patients, payors and other customers.

    + **Location:** Sarasota Memorial Hospital - Sarasota, FL - ONSITE

    + **Status:** PRN, As Needed

    + **Schedule:** Days will vary, 8:00 AM - 5:00 PM

    + **Weekend requirement:** occasional

    + **On Call:** No

    Education:

    + Required High School Graduate or Equivalent GED

    + Preferred Technical Diploma – Medical Specialization

    Experience:

    + Required 1 year Customer Service

    + Required 1 year Insurance Verification

    + Required 1 year Medical Related field
