    Palliative Care Coordinator Qualifications:

    + Must possess a current, unencumbered LPN License

    + Experience in scheduling in a healthcare organization or unit

    + Receive and promptly process palliative care referrals and/or inquiries for services provided by Crossroads

    + Work in cooperation with the call center for incoming or received referrals.

    + Adept knowledge of the Palliative Care program and ability to explain to the fullest extent palliative care utilization and services available.

    + Maintain accurate and detailed records of all information calls and referrals.

    + Schedule and assign all palliative patients with designated Palliative Care Nurse Practitioners as appropriate.

    Palliative Care Coordinator Schedule & Benefits:

    + Schedule: Monday to Friday, 8:30 AM - 5:00 PM
