Licensed Practical Nurse - Discharge Throughput Full-time Day Shift
Position Summary
The Discharge Throughput LPN is responsible for facilitating discharge callback protocols for low risk patients discharged to the home. The LPN will work under the Discharge Throughput Nurse Navigators to maintain a high level of quality and safety as they engage with patients within the SBM system. They will collaborate with the patient’s care team as needed while also utilizing their knowledge base to educate, advise, and assist patients to ensure a smooth transition from hospital to home.
_This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS)._
_For the duration of the NY HELPS Program, this title maybe filled via a non-competitive appointment, which means no examination is required but all candidates must meet the minimum qualifications of the title for which they apply. At this time, agencies may recruit and hire employees by making temporary appointments. In May 2024, if a temporary NY HELPS employee is satisfactorily performing in the position, the appointment will be changed from temporary pending Civil Service Commission Action to permanent non-competitive and the official probationary period will begin._
_At a future date (within one year of permanent appointment), it is expected employees hired under NY HELPS will have their non-competitive employment status converted to competitive status, without having to compete in an examination. Employees will then be afforded with all of the same rights and privileges of competitive class employees of New York State. While serving permanently in a NY HELPS title, employees may take part in any promotion examination for which they are qualified._
Duties of a Discharge Throughput LPN
+ Generate patient readmission risk lists and place patient call backs focused on the low risk patients
+ Gauge patient understanding of post-discharge instructions
+ Assist in coordinating follow up care including accessing prescribed medications, scheduling follow up appointments, and facilitating home care
+ Utilize interdisciplinary teamwork to obtain relevant information and connect patients with appropriate resources
+ Identify when calls should be escalated to Discharge Throughput Nurse Navigators and facilitate transition
+ Effectively document call backs in the required format and maintain call back log
+ Adhere to protocols, guidelines, and policies for the follow up of patients at risk for readmissions by collaborating with Discharge Throughput Nurse Navigators
+ Participate in all Discharge Call Back program developments and quality improvements to achieve improved patient outcomes
+ Maintain all aspects of standard of care for Stony Brook Discharge Call Back initiatives
+ Facilitate communication between and among the patient and providers and act as a liaison with the Nurse Navigator on behalf of the patient
+ Utilizes a collaborative, interdisciplinary approach to maximize the patient experience
+ Works closely with leadership on joint projects to improve and maintain patient workflow processes, standard of cares, and quality data
+ Collaborate with patient education to maintain updates of all print and electronic materials pertaining to Pt discharge/follow-up Educational materials
+ Develop and/or promote patient and family Resources in the SBM
+ Perform all other duties as assigned by management
Working Conditions:
Working conditions may involve exposure to infectious disease, blood, body fluids and/or tissue as well as other unpleasant elements such as accidents, injuries and illness. May be exposed to and be required to handle varying unpredictable, emergency and crisis situations. Occasionally subjected to irregular hours and pressure due to multiple calls and inquiries. This position is classified by OSHA as Category 1.
Qualifications
**Required:** Must possess or be eligible to obtain a license to practice as a Licensed Practical Nurse in New York State.
**Preferred:** A minimum of 3 years of clinical related experience. Advanced knowledge of Microsoft Excel, Word, Power Point skills. Ability to work independently prioritizing initiatives within timelines in an organizational manner
** As part of the onboarding process, we require a copy of your current Basic Life Support (BLS) certification. This allows us to accurately track expiration dates and ensure compliance.
If your BLS certification is set to expire within 90 days of your start date, you will be scheduled to attend a BLS training session during your nursing orientation.
* Acute care is defined as medical services provided for illnesses that are short-term (usually less than 30 days), intense, and have a relatively severe to very severe course, requiring knowledge and expertise in the vigilant monitoring of subtle changes in the patient’s condition, as well as ability to quickly recognize and identify potential problems and conditions resulting from a previous disease.

The salary range (or hiring range) for this position is $45081 / year.
The above salary range represents SBUH’s good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidate’s validated years of comparable experience.
