    Responsibilities

    **Are you a compassionate and skilled Social Worker looking to impact lives?** Join our dynamic Care Coordination team and provide vital support to patients and families in our hospital and emergency department.

    In this role, you will:

    + Provide crisis intervention, psychosocial assessments, and counseling.

    + Advocate for patients and families, connecting them with essential resources.

    + Facilitate complex discharges and transitions of care.

    + Collaborate with an interdisciplinary team to ensure optimal patient outcomes.

     

    Shift: Scheduled Days and Hours are Flexible, but must include coverage on Saturday and Sunday

    Qualifications

    We're looking for someone with:

    + Master's degree in Social Work (CSWE accredited)

    + 2+ years post-MSW experience (5+ years preferred)

    + Oregon LCSW or CSWA working towards licensure (LCSW required within 2 years)

    + Excellent communication, problem-solving, and teamwork skills

    Roseburg is located on Interstate 5 in Southern Oregon's scenic Douglas County. The community is nestled in a lush forested region known as the 100 Valleys of the Umpqua.     Starting hourly rate of at least $38.72/hr!** **Student loan repayment program as well as relocation assistance offered!

    Pay Range

    $38.72 - $56.14 /hour
