Additional Information About the Role

Shift Information: Monday thru Friday 8-4:30 (4 weekends per year Saturday and Sunday 8-4:30- they take 2 scheduled days off during the week for each weekend worked)

+ Home Office Location: - BJC Home Care - 670 Mason Ridge Center Dr Ste 300 -Saint Louis, Missouri 63141 – Position will cover the Chesterfield and St Charles area as part of the West Pod Team

+ Case Load: You will be responsible for a caseload of 45-55 patients in patient’s residences, nursing homes and assisted living facilities.

+ Job Responsibilities: Meeting with patients/families, spiritual support needs, counseling support needs, admission and discharge paperwork.

+ Preferred skills or required work experience: home health, hospice or palliative experience is not required, but a bonus. Additionally, this is a very independent position, prior professional experience is preferrable.

Overview

**BJC Home Care** offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.

Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. The Hospice staff are sensitive to the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and their families. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. Our Hospice services include alternative therapies such as music, art and massage therapy. Our Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity.

Role Purpose

Provides spiritual care, short term counseling, worship leadership, education and other religious support to patients, families and staff as members of the interdisciplinary care team.

Responsibilities

+ Provides direct spiritual care to patients and families.

+ Provides consultation to other members of the care team regarding spiritual issues and resources in patient and family care and regarding ethics.

+ Provides direct pastoral care to staff and physicians, as needed, complementing staff support services provided by others.

+ Contributes to the field of professional spiritual care by conducting and reporting research, publishing and providing workshops at professional meetings.
