1. Oversees all entries into OPWDD’s Incident Report Management software for the agency within 24 hours of reporting; oversees all documentation related to incident management including 149 reports and correspondence with regulatory bodies, families, and other agencies.

2. Ensures that all applicable notifications are completed whenever OPWDD Forms 147 and 148 are initiated. Generates these forms within 24 hours as required and provides hardcopy notifications and letters to regulatory agencies and families as required/requested.

3. Collects and/or directs the collection of data relevant to the reporting of abuse/neglect allegations and significant incidents to the Justice Center and the Office for People with Developmental Disabilities (OPWDD).

4. Conducts investigations on reportable injuries, and allegations of abuse, neglect, or any other significant incidents according to OPWDD and Justice Center guidelines and 624 & 483 regulations, as assigned. Prepares written investigative summary (utilizing the 149 format) with appropriate recommendations, in consultation with relevant administrators and Human Resources. Enters all information related to investigation process and findings data into Incident Reporting Management Application (IRMA) software.

5. Ensures that all investigations are completed within timeframes established by the Justice Center, OPWDD, 624 & 483 regulations and the Developmental Disabilities Regional Office (DDRO).

6. Collaborates with the Director of Quality Improvement on the development of policies and procedures to comply with all regulations and requirements of the Justice Center, OPWDD, 624 and 483 regulations and the Hudson Valley DDRO.

7. Participates in meetings with clinical teams and/or other departments to discuss and/or resolve issues or problems in the areas of incident management and disseminates information to all appropriate Agency staff.

8. Keeps abreast of appropriate regulatory Agency requirements related to incident management. Reviews proposed and adopted changes and recommends policy and/or procedure changes consistent with regulatory changes. Informs applicable staff of these requirements/changes. Stays abreast of state-of-the-art risk management and incident reporting trends, management process techniques, and measurement tools through seminars, webinars and written communications with OPWDD.

9. Acts as a liaison with families and “qualified persons” as defined by Jonathan’s Law regarding investigation policies, procedures, and reporting of findings.

10. Communicates with DDRO, Justice Center, and OPWDD staff regarding potential incidents, allegations and reportable injures.

11. Participates in various administrative and other meetings as required at the Richmond Community Services corporate offices and at other locations.

12. Ensures investigation and documentation of complaints that do not meet the criteria of “reportable” for the Justice Center or DDRO. Maintains a file of non-reportable complaints with supporting rationale.

13. May conduct orientation and other trainings related to incident reporting and incident management, as well as Justice Center, OPWDD, and risk management regulations, for various levels of staff and newly hired employees, as assigned.

14. Prioritizes job tasks to ensure timely and accurate completion of required work. May be assigned other tasks and duties reasonably related to job responsibilities.
