SUMMARY OF POSITION:
The Specialty Clinical Pharmacist is responsible for providing comprehensive patient care to improve pharmacotherapy outcomes for patients living with serious health conditions requiring complex specialty drug therapies. The Specialty Clinical Pharmacist is responsible to educate and support patients prescribed specialty medications, as well as develop and coordinate patient care within the clinical care team. This individual will coordinate activities and lead resolution of issues between the pharmacy, provider, insurance and patient for specialty medications in compliance with regulatory standards and may interact with healthcare providers in the clinic, specialty technicians, prior authorization specialists. The position reports to the Specialty Pharmacy Clinical Coordinator. Responsibilities include but are not limited to prospective evaluation and ongoing monitoring of specialty drug therapy, assessing patient needs, making patient specific recommendations, adjusting care according to patient response, providing medication education, ongoing medication adherence outreach, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. The Clinical Specialty Pharmacist collaborates and maintains excellent working relationships with peers, interdisciplinary colleagues and other healthcare providers to provide exceptional patient care.
Other opportunities include involvement with research projects, routine precepting of pharmacy learners, and teaching of peers and interdisciplinary colleagues.

MINIMUM QUALIFICATION(S):
+ Bachelors of Science degree in Pharmacy or Doctorate Degree in Pharmacy required.
+ ASHP-Accredited PGY-1 Pharmacy residency or three (3) years of relevant clinical pharmacy experience required.
+ Active and valid PA licensure or eligibility as a Pharmacist required.
