This position is a part-time RN Case Manager for the Solid Tumor patient population. It will be three 8-hour shifts per week from 0830-1700, sharing the position with another RN Case Manager. Preferred Peds HemOnc and RN case management experience. This role includes telephone triage, imaging management, insurance and patient care coordination.

JOB SUMMARY:

Peckham Center Case Manager coordinates provision of care across the continuum for pediatric patients with a hematology and/or oncology diagnosis. The Case Manager advocates for the patient and family through triage, education, and navigation of insurance authorization requirements, as appropriate, to the nursing role. The Case Manager identifies, accesses, and allocates resources to meet the goals of care, as identified by the multidisciplinary team, in partnership with the patient and family.

MINIMUM QUALIFICATIONS:

Bachelor's Degree

3 years of experience

California RN License

BLS for Healthcare Providers, issued by AHA

Must qualify for California Children's Services (CCS) Paneling

Excellent oral and written communication skills

Able to manage large call volumes and triage patient care needs appropriately

Experience working with children and adolescents with hematology and/or oncology diagnoses

PREFERRED QUALIFICATIONS:

Master's Degree

5 years of experience

Bilingual - English/Spanish

The current salary range for this position is $59.73 to $97.42
