    Job Summary and Qualifications

     

    The medical social worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. The social worker acts as a liaison between the patient and community resources/post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The social worker facilitates patient movement across the continuum of care and identifies, addresses and tracks barriers to patient throughput.

    What you will do in this role:

    + You will perform continuous assessments and evaluations to ensure patient is progressing towards desired outcomes.

    + You will collaborate and consult with physicians on patient's progress and discharge planning needs.

    + You will perform utilization review ensuring admissions meet criteria for appropriateness of care and medical necessity.

    + You will coordinate the discharge planning process ensuring involvement of all members of the healthcare team. Counsels with patients and family members in decision making and in meeting psycho social needs of the patient.

    + You will collaborate with personnel at other facilities to coordinate smooth and effective patient transfers and transitions.

    What qualifications you will need:

    + State of TN LCSW Licensure preferred.

    + Bachelor's Degree in Social Work required Master's Degree preferred.

    + 1 year in Healthcare with demonstrated understanding of acute care social service issues

     

    Parkridge East Hospital is a 120+ bed facility in East Ridge. We provide a full range of medical and surgical services. These services include orthopedics and robotic surgery. We have a 24/7 Emergency Department. This ED is recognized by the Society of Chest Pain Centers as an Accredited Chest Pain Center. The facility also offers obstetrical care and complex gynecological procedures. We also hold a Level III Neonatal Intensive Care Unit. Specialized providers in the NICU include neonatologists, neonatal nurses and respiratory therapists. These staff are on site at all times. Visit us for excellent general and OB care!
