    Job Description:

    A Brief OverviewThe Restorative Care Assistant (RCA) is responsible for providing nursing restorative care to ensure residents attain or maintain the highest possible physical, mental and emotional well-being without declines unless the decline is documented as unavoidable. The RCA provides care to residents that is designed to strengthen muscles, assist with mobility, promote independence, prevent functional declines. Works in accordance with long term care policies and procedures, resident's plan of care, current standards of care and practice and applicable state/federal regulations.What you will do

    + Performs dressing, grooming, and bathing per individualized resident restorative care plan.

    + Exercise/Restorative/Rehabilitative Care • Assists with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, B/R, W/C, lifts, etc. • Perform restorative/rehabilitative care and services (ambulation, gait training, range of motion, self-feeding and other activities of daily living), with residents as indicated in individual care plans and per resident preference. • Document each treatment provided, participation, refusal of treatment, reason(s) treatment could not be provided and progress notes as required. • Contribute knowledge of resident's condition/needs/progress to nursing and therapy staff and to the care planning process; provide written reports and attends meetings as requested. • Monitor use and condition of adaptive equipment/assistive devices arranging for repair as needed. • Assist in maintaining therapy area and equipment in clean, safe, functional condition. • Explain restorative/rehabilitative services to resident/family members as needed. • Participate in interdisciplinary and other facility meetings as assigned. • Follow facility procedures for admission, discharge and transfer of residents. • Performs Active Range of Motion (AROM) and Passive Range of Motion (PROM), Therapeutic Strengthening exercise in a safe manner to residents as assigned by the Therapist or Restorative Nurse in their overall plan of care. • Assists residents with applying and removing splints or prostheses. • Encourages the residents to perform tasks for him/herself as appropriate. • Monitors and assists with the performance of Activities of Daily Living (ADLs) to promote resident involvement to the highest extent possible without resident decline unless the decline is documented as unavoidable. • Practices good body mechanics while delivering restorative nursing care to prevent injuries to self or to others.

    + Elimination • Assists residents to and from the bathroom. Keeps incontinent residents clean and dry at all times. • Uses appropriate incontinence product for each resident as specified in the care plan. • Provides perineal care as required. • Monitors and documents output.

    + Vital Signs and Weights • Weigh residents monthly and take vitals as needed.

    + Documents/Records information • Enters, transcribes, records, stores, or maintains information in written or electronic/magnetic form.

    + Maintains Infection Control • Decreases likelihood of possible transmission of pathogens by utilizing infection control principles.

    Qualifications

    + H.S. Diploma or General Education Degree (GED) Required

    + 1-3 years Previous experience in long term care. Preferred

    + 1-3 years Previous training in restorative care procedures. Preferred

    + Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.

    + Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.

    + Actively looking for ways to help people.

    + Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.

    + Monitoring/Assessing performance of yourself, other individuals, or organizations to make improvements or take corrective action.

    + Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.

    + Demonstrates the desire and ability to work with different personalities to accomplish a common goal.

    + Listens to staff and other sources; Ask questions to ensure he/she understands the staff member(s) ideas and perspectives; Shows listening with body language and action; Is perceived as easy to talk to about things.

    + Creates an environment where staff participate in decision making and changes that affect the work they do. Ask staff for input and integrates ideas. Recognizes staff for ideas and suggestions for improvement.

    + Effectively demonstrates the ability to coordinate multiple activities and documentation effectively.

    + Preferred training in restorative care procedures.

    + Being aware of others' reactions and understanding why they react as they do.

    + Actively looking for ways to help people.

    + Adjusting actions in relation to others’ action.

    + The ability to see details at close range (within a few feet of the observer).

    + The ability to exert maximum muscle force to lift, push, pull, or carry objects.

    + Demonstrates an ability to relay information in a timely and appropriate manner.

    + Demonstrates the ability to correctly document information for assessing a plan of care for each resident.

    + The ability to demonstrate a working knowledge of computer, gathering and inputting data.

    + Certified Nursing Assistant Upon Hire Required

    + Basic Life Support Healthcare Provider - American Heart Association within 60 Days Required

    + Child Abuse Iowa Mandatory Reporter - Iowa Department of Human Services within 180 Days Required

    + Dependent Adult Abuse Mandatory Reporter - Iowa Department of Human Services within 180 Days Required

    Qualifications:

    Active status on the Direct Care Worker's Registry is required.Maintains current and active status on all required training and education as outlined by corporate policy is required.
