What You’ll Do:
+ Assist family meetings to clarify goals of care, code status and, when appropriate, transition to hospice.
+ Assist with or facilitate family meetings to clarify goals of care, code status and when appropriate, transition to hospice.
+ Coordinate care among both internal and external clinicians and health care providers.
+ Provide on call services via phone for hospice patients, both in the community and at the hospice house.
+ Participate in care coordination meetings as needed, including mornings ICU rounds, St. Mary’s oncology, hospice house and palliative care interdisciplinary team meetings.
+ Act as a resource to staff, patients, family members and attending providers regarding pain and symptom management.
+ For patients at the Hospice house, serve as attending or covering provider and is responsible for ordering medications, treatments, and testing as appropriate.
+ Perform palliative care initial consultations and follow up visits in the community or acute care setting as requested by the primary care team.
+ Provide care based on current clinical practice guidelines and evidence-based practice.
What You’ll Bring:
+ Master’s degree in nursing and certification by the American Academy of Nurse Practitioners (AANP).
+ One year of recent clinical nursing experience required.
+ Current R.N. license in the State of Maine.
+ Current CPR Card.
+ Must have a reliable and insured automobile and have a current Maine driver’s license.
+ Independent malpractice insurance strongly advised.
