Job Description:
The Population Health Network Manager provides a lead role in overseeing the development and management of our population health network. Responsible for building relationships with clinic organization leaders, healthcare providers, payers, and community organizations to improve patient outcomes and reduce healthcare costs through execution of population health management strategies. Oversees and coordinates the work of various groups throughout the network. Ensures access to data, resolves client concerns, holds clinic organizations accountable, and builds engagement and loyalty.
Position Details:
This position will cover the St. George and Peaks geographies.
This is a M-F 8-5 hybrid position with occasional in-office days in SLC and Murray. Also, quarterly travel to St. George, CO, and MT. Estimated 8-10 days out-of-town travel per quarter. There is also potential for geographies to change over the next year but travel would only be reduced, not increased.
Essential** **Functions
+ Builds and maintains effective business relationships with network partners to facilitatetimely and accurate exchange of information and data necessary for effective population health management and meeting network partner needs
+ Facilitates deployment of new and existing payer contracts with network partners. Recruits new community partners to risk arrangements.
+ Educate groups about population health products, strategies, and how to be successful in various payment models, includingeducation about risk adjustment, cost, and quality topics
+ Reviews performance reports with clinic leaders and providers relevant to their practices and identify areas of opportunity andnavigate data tools to monitor and analyze population health data to identify trends, develop strategies to address gaps in care, and provide solutions for actionable clinic and provider opportunities.
+ Leads and facilitates implementation of Population Health services in collaboration with team members and partnerships.
+ Manages the development, maintenance, and distribution of materials that assist clinicians and their teams to interface more efficiently with the administrative processes.
+ Collaborates with internal stakeholders to identify and implement population health initiatives that align with the organization's goals and objectives.

Required** **Qualifications
+ Demonstrated professional experience in population health.
+ Demonstrated experience in the health care industry.
+ Demonstrated experience managing a program or team in a professional level position.
+ Excellent communication and interpersonal skills, with the ability to build and maintain relationships with internal and external stakeholders.
+ Demonstrated ability to manage projects and teams, with a focus on achieving goals and objectives within established timelines and budgets.
+ Knowledge of healthcare regulations and compliance requirements related to population health management.
Preferred Qualifications
+ Experience with Risk Arrangements
+ Experience transitioning employees from FFS (fee-for-service) to Fee-for-Value or Value-Based Care
+ Experience with Payment Models
+ Experience with Risk Adjustment
+ Experience with ACOs (Accountable Care Organizations)
+ Experience with MCOs (Managed Care Organizations) or Managed Care
+ Experience with CINs (Clinically Integrated Networks)
Physical Requirements:
+ Frequent interactions with providers, colleagues, customers, patients/clients and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
+ Manual dexterity of hands and fingers to manipulate complex and delicate supplies and equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
+ Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$43.67 - $67.41
