    Summary:

    Works with the System Director of Pharmacy Services, system and site pharmacy leadership in the establishment and maintenance of a comprehensive, patient focused clinical pharmacy services program specializing in Hematology/Oncology and General Infusion Therapy. Incumbent coordinates and actively participates in various programs and activities within the Oncology and General Infusion Center and within the scope of the Clinical Pharmacy Services Program. Works collaboratively with Oncologists, non-physician practitioners, nursing staff, and infusion center/Pharmacy leadership in the optimization of patient care by promoting rational, safe, and cost effective drug therapy by developing and providing clinical services in hematology/Oncology and General Infusion therapy. Conducts drug therapy monitoring and drug formulary reviews, implements drug therapy interventions to reduce adverse events. Participates in various research projects including oncology and may conduct pharmacy practice research. Provides educational programs and training for nurses, pharmacists and pharmacy staff, medical and pharmacy residents, physicians, non-physician practitioners, other members of the healthcare team as well as patients and their families. May be required to provide non-clinical/operation support Precepts pharmacy residents and students as required from across the system. May be required to provide consultations and/or clinical pharmacy services for inpatient hematology/oncology patients and/or other system infusion sites and inpatient facilities as needed. Assists System Director of Pharmacy, system, and site pharmacy leadership in maintenance of readiness by regulatory agencies and in ongoing compliance with all statutory regulations.

    Responsibilities:

    EDUCATION AND EXPERIENCE

    + Doctor of Pharmacy degree (PharmD) with PGY1 Pharmacy Residency in an acute care setting required. PGY2 Pharmacy Residency in Oncology or Hematology/Oncology preferred. Current PGY2 residents in oncology or hematology/oncology accepted for application. Board Certification in Oncology Pharmacy preferred, and required within one year of hire, CDTM required (see below), or

    + PharmD with completion of a PGY-1 Pharmacy Residency in an acute care hospital. PGY1 rotations in Oncology/Hematology preferred. Plus, one year of fulltime (or equivalent) experience in Oncology or Hematology/Oncology required. Board Certification in Oncology Pharmacy preferred, and required within one year of eligibility. CDTM required (see below)

    + Residency requirements

    o All Pharmacy Residencies require American Society of Health-System Pharmacists (ASHP) accreditation or candidate status

    o All residencies must be verified for completion through presentation of certificate of completion from residency site and/or the database of the ASHP

    o Candidates currently enrolled in an accredited or candidate status pharmacy residency program are eligible to apply. Evidence of residency completion must be available prior to start date

    + Experience Requirements

    + o Relevant past experience must have been obtained in entirety within three years of application

    + o Required full time equivalent experience may be accrued over two year period

    + Board Certification in specialty area

    o Must be obtained through nationally recognized organizations such as the Board of Pharmacy Specialties (BPS)

    o Ongoing maintenance of board certification required

    + Collaborative Drug Therapy Management (CDTM) Requirement

    o Candidates must be certified/registered in NY State to participate in CDTM at the time of hire or within 6 months of start date.

    o PGY1 residency year counts as a year of clinical experience

    o PGY2 residency counts as second year of clinical experience

    + Within one year of hire, will be required to complete a certificate program in pharmacogenomics (e.g. AACP Applied Pharmacogenomics Certificate Program) to facilitate participation in a personalized medicine approach.

    + Additional certificate programs or board certifications may be required in response to advancement in treatments and modalities or change in scope of services

    + Preceptor Requirement

    o Within 12 months of hire, maintains compliance with preceptor qualifications set forth by the American Society of Health-System Pharmacists (ASHP)

    + Licensure and Academic requirements

    o PharmD degree must be obtained from an ACPE accredited US school of pharmacy or a foreign school recognized by NABP, BPS and/or ASHP

    o Valid license and current registration to practice Pharmacy in New York State

    KNOWLEDGE, SKILL AND ABILITY

    + Ability to analyze/interpret and utilize results of various laboratory and microbiology tests and apply them to patient care in a clinical practice setting

    + Excellent interpersonal skills and the ability to convey educational material in an understandable manner to patients and their families.

    + Ability to coordinate activities of multiple personnel and supervise their daily activities

    + Ability to use hospital based information systems including Pharmacy, Laboratory, Patient Accounting and office systems

    + Ability to operate ancillary patient care information systems in conjunction with operation of patient testing equipment

    + Ability to communicate effectively with Physicians, Physicians Assistants, Nurse Practitioners, Nurses and other allied health professionals

    WORKING CONDITIONS:

    + Travel to various locations for clinical assignments, meetings, and educational programs required

    + Works unusual hours, days and shifts to accommodate needs of the educational activities of the Clinical Pharmacy Services Program

    + Works in a clinic setting with considerable periods of standing and sitting

    ENVIRONMENT

    + Normal heat, light space, and safe working environment; typical of most office jobs

    + Occasional exposure to one or more mildly unpleasant physical conditions

    + Occasional exposure to communicable diseases of pathogens through patient contact
