    Hennepin Healthcare (HCMC) has a current opening for a *Staff Nurse*in Home Health.

     

    This position is a 1.0FTE (10 shifts/pay period), 8-hour/Flex 40 shifts, day shift and every 4th weekend.

     

    *RESPONSIBILITIES:*

    Within the context of the Hennepin Healthcare System Relationship-Based Care practice model, and under general supervision, the registered nurse provides and coordinates nursing care by:

    * Protecting, promoting wellness and optimizing health

    * Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care

    * Alleviating suffering through the treatment of the human response

    * Partnering with the patient, their family and significant others

    * Documenting interventions and an individualized plan of care

    * Educating patient on their disease process

    * Assuring continuity of care

    * Educating patient and family in preventative care which promotes health and well being

    */Minimum Qualifications:/*

    * New Grad OR 2 years of professional home visiting workexperienceas a LPN or RN

    * Must have a valid driver’s license and the availability of an insured vehicle to travel to patient's homes or agreed upon meeting places

    * Graduation from an accredited school of nursing

    * Possession of a valid license as a Registered Nurse issued by the State of Minnesota

    * Public Health/ Home Care professional nursing (RN) experience

    * Public Health Nurse certification

    * EPIC and OASIS experience
