    Job Title: Geriatric Case Manager

     

    DEPARTMENT: Home Care

     

    STATUS: Full Time Days

    SUMMARY

    The RN - Geriatric Case Manager, Home Care is responsible for the delivery of comprehensive nursing care to a set of assigned patients thru the RRH Geriatric Service Verification program. This involves the pre-surgical assessment of eligible patients in the GSV program. The RN will work with the patient, family, Geriatric APP team to recognize post discharge needs and the development, implementation and evaluation of an appropriate Plan of Care. He/She will make changes in response to evaluation & assessment of patient needs. The RN Geriatric Case Manager identifies appropriate interdisciplinary services needed, coordinates those services and supervises the delivery of care.

    RESPONSIBILITIES

    • Coordinate/participate in interdisciplinary team meetings/patient care conferences.• Supervises and evaluates care provided by Licensed Practical Nurses and/or home health aides in the performance of his/her patient care duties.• Accurate, thorough and complete documentation per agency standard and program standard. Participates in utilization review and/or continuous quality improvement activities as requested.• Attends required staff meetings, in-services and/or supervisory conferences.• Develop strong relationships & integration into the system Geriatric Service programs and work towards expansion within the system.• Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures• Regular and reliable attendance is expected and required. • Works independently in coordinating and collaborating with members and providers, resulting in improving member and community health.• Performs other job-related duties as assigned by management. • Mentors new staff & assists with coaching whenever necessary. Serves as an informal leader, role model and resource to other staff in a positive clinical leadership role. • Offers process improvement suggestions and participates in the solutions of more complex issues/activities.

    Required Qualifications:

    • Registered Nurse license in New York State.

    • Bachelor’s Degree in Nursing

    • Valid NYS Driver’s License

    Preferred Qualifications:

    • 1 year of nursing experienced preferred.

    • Prior home health, clinical and direct patient care experience preferred.

    EDUCATION:

    BS: Nursing (Required), MS

    LICENSES / CERTIFICATIONS:

    BLS - Basic Life Support - American Heart Association (AHA)

    PHYSICAL REQUIREMENTS:

    M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

    $75,712.00 - $100,880.00

    The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
