Summary: Provide coordination of all client flow and case management. Oversee all MST / MW SCM / MW PSH Program case managers and provide supervision weekly. Ensure adherence to all program policies and procedures, including verifying all potential program participants.

Primary Responsibilities:
+ Manage the operational and fiscal activities of the program/department to include: staffing levels, budgets, and financial goals.
+ Recruit, manage, develop, and evaluate staff by providing timely mentoring, training, and performance management. Complete required paperwork in accordance with company policy.
+ Ensure adherence to policies and protocols for programs serving chronically homeless individuals, including guidelines for meeting attendance, client transportation to appointments, etc.
+ Ensure all participants meet the HUD and DPH requirements for entry into the program and that all required documentation is included in the client file.
+ Conduct initial service needs assessment with all clients entering the program. Develop an Individual Service Plan with each participant within three days of entry into the program.
+ Work closely with Shelter staff, Relapse Prevention Managers, and Supported/Affordable Housing staff to identify new program participants.
+ Maintain a caseload of up to 15 clients and maintain daily contact with clients actively involved in the program.
+ Establish and maintain a regular contact schedule with all participants for a period of at least 24 months.
+ Ensure accurate statistical data is maintained and reported monthly.
+ Maintain accurate program rosters for billing and reporting purposes.
+ Outreach to treatment programs and other community-based resources to establish contacts for client referral; keep a detailed log of all referrals made.
+ Coordinate staff coverage of crisis intervention (24-hour phone response) and provide coverage as needed.
+ Review case manager notes in CSG Engage and ETO regularly to ensure compliance with program specifications.
+ Ensure compliance with program/department, agency, and/or funder requirements and policies & procedures.
+ Engage all clients by understanding and addressing their needs whether within or outside the scope of work.
+ Attend & participate in engagement team meetings as requested and communicate effectively with clients and staff in other areas.
+ Maintain confidentiality of client, employee, and agency information in accordance with federal and state laws and funder requirements.
+ Ensure compliance with program/department, agency and/or funder requirements, as well as, SMOC policies & procedures.
+ Other duties as assigned.
Knowledge and Skill Requirements:
+ At least 5 years supervisory / management experience preferred.
+ Excellent computer skills including intermediate to advanced proficiency in Word, Excel, and Powerpoint. Strong understanding of databases necessary.
+ Proven organizational skills and strong written and verbal communication skills.
+ Prior work with the homeless population and substance abuse required.
Organizational Relationship: Directly reports to the Director of Supported Housing. Direct reports to this position are MST, Metro-West Supported Case Management, and MW Permanent Supported Housing Case Managers.
Physical Requirement: Must be able to travel across Metro-West area via personal vehicle. Must be able to transport clients in personal vehicle and acompany to appointments/interviews. Must be able to sit or stand for prolonged periods of time. Must be able to operate a computer and complete extensive paperwork.
Working Conditions: Some travel required. Twenty-four-hour cell phone coverage is required for crisis intervention. As part of the responsibilities of this position, the Case Management Supervisor will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.
Remote Work Option: Remote work is permissible in some positions at SMOC depending on the key functions and responsibilities. The Case Management Supervisor, Day Center position is eligible to work from home 0-20% of the week in scheduling coordination with the department manager.
