Salary Range: $21.00 - $28.00 Hourly

Role and Responsibilities

This position coordinates and ensures compliance to the Face - to - Face client visit with their primary care physician requirement. This role involves coordination and follow-up with clinical staff and physicians.

Essential Functions

+ Process Workflow in HCHB to produce Face - to - Face documents for physician signature , in accordance with the regulations.

+ Run reports and monitor compliance of returned forms and review for adequacy of documentation to meet the intent of federal regulation s .

+ Coordinate with physicians/physician ’s office staff and HCR clinical staf f , as needed , to procure forms not completed .

+ Review returned documentation to ensure clinical components of the requirements are met.

+ Process documents through HCHB to allow billing to occur .

+ Other duties as assigned.

Education Requirements

+ Associate's degree required.

Qualifications and Requirements

+ Current, valid LPN license from NYS Department of Education required .

+ This role will require critical thinking skills, strong organizational skills , and excellent communication skills .

+ There is also a need for clinical oversight/interpretation when reviewing the documentation.

+ Must possess basic computer skills – Email, Internet, Microsoft Office.

Work Environment

The Face-to-Face Coordinator LPN is primarily in an office setting and may be exposed to outdoor conditions.

The working conditions are classified as sedentary work:

+ Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
