    1 weekend per month. 8.5-hour shifts between 0730 and 2100. Additional hours may be available upon request.

    The **Medication History Technician** will **obtain and document medication histories** in the electronic medical record in an accurate and timely manner in compliance with standard procedures, the Joint Commission, and other regulatory standards.

    Your Exceptional Skills and Qualifications

    • Performs thorough interviews with patients and family members with appropriate professional language.

    • Conducts telephone interviews with outpatient pharmacies, long-term care facilities, or other locations that may provide medication history with appropriate telephone etiquette.

    • Interprets and enters “Prior to Admission” medication history into the electronic medical record in an accurate and timely manner.

    • Coordinates responses and resolutions to issues with appropriate internal and external parties.

    • Regularly and appropriately communicates with supervising pharmacists for any questions and additional work assignments.

    • Accurately tracks interventions and productivity using department’s process.

    • Maintains proficiency with departmental information system technology and documentation systems.

    • Maintains contemporary professional knowledge base.

    • May represents department on various hospital committees.

    • Participates in evaluation of quality service, development, and implementation of improvement.

    • Communicates with other hospital employees and departments as necessary.

    • High School Diploma or GED (Required)

    • One of the following is required CertIfications/Licensures: (Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA), Licensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA). If licensed as Pharmacy Intern (IPLA) you do not need to be nationally certified with PTCB or NHA.)

    • within two (2) years from hire CertIfications/Licensures: (Pharmacy Technician Certification Board (PTCB) National Certification or National Healthcareer Association (NHA) (ExCPT))
