Responsibilities
Provides social work services for facilitating solutions to the medically related psychosocial/environmental problems of patients and families. Ensures the continuity of patient care by implementing interventions which may include initial assessment, identifying mental health disorders, brief behavioral health interventions , information and referral, during hospitalization and/or outpatient treatment. May provide educational support to staff. Maintains positive working relationships with vendors and external agencies to access services for patients. The individual must possess the ability to clinically assess and treat emotional mental health issues. The individual will also be able to diagnose under the direct supervision of a LCSW social worker. This LMSW position will assess data reflective of the patient's status and appropriately interpret information relative to the patient's age-specific needs is required.
Qualifications
Master's degree in Social Work. One year of current experience in a clinic, hospital, medical setting, or healthcare field preferred. Must have knowledge of adjustment to medical diagnosis and chronic disease. Strong communication skills are needed to interact with patients, family members/ care givers, providers and allied health staff. Must have psychosocial diagnostic skills and ability to coordinate transition of care. Active license through Arizona State Board of Behavioral Health Service Examiners; Licensed Master Social Worker (LMSW) required.
Transplant experience preferred.
Medical experience required. LCSW preferred. LMSW required.
**Job Summary:** We are seeking a compassionate and detail-oriented Kidney Waitlist Social Worker to join our transplant team. The ideal candidate will be responsible for managing the kidney waitlist, providing support and education to patients, and collaborating with healthcare professionals to ensure the best outcomes for transplant candidates. The candidate must be a team player while also being able to make independent decisions. They will conduct thorough evaluations of patients, including social work visits, caregiver vetting, and compliance checks. The role involves coordinating with healthcare providers, patients, and other stakeholders, sending updates, and managing communications. The social worker will provide psychoeducation and support to patients on the waitlist, including information on what to expect during the transplant process, financial considerations, and caregiver requirements. They will ensure all activities comply with relevant regulations and guidelines, maintaining patient confidentiality and data security. Collaboration is key, as the social worker will work closely with physicians, surgeons, nurses, and other disciplines as well as social work colleagues, and must be willing to speak up in large groups and confidently provide information. Handling difficult conversations with patients and crucial conversations regarding transplant readiness, including providing unfavorable information, when necessary, is an essential part of the role. The social worker will identify exacerbated mental health needs versus adjustment to chronic illness, critically think through chronic disease and its impact on life and comfortably provide medical psychoeducation. Additionally, they will understand opportunities for innovations and develop transplant plans and rebuilding plans.
Preferred Qualifications:
+ 2 years’ experience in a medical setting. Transplant experience is strongly preferred; dialysis experience preferred.
+ Able to adjust to medical diagnosis and chronic disease. Strong communication skills are needed to interact with patients, family members/caregivers, providers, and allied health staff 15.

$68,099 - $102,232 / year; Education, experience and tenure may be considered along with internal equity when job offers are extended.
