    Job Summary and Qualifications

     

    Inpatient trauma care relies on a multitude of interrelated surgical and medical specialties, diagnostic tests, therapeutic interventions, and allied health services to reduce mortality and morbidity from severe injuries. The complexity of this care can often make mental health and social determinates difficult to identify and treat. The trauma social worker will focus on identifying these areas of need and helping coordinate care with community resources.

    Major Responsibilities:

    + Perform thorough assessments and develop appropriate plans of care for patients and their families who have experienced traumatic injuries which includes discharge planning and psychosocial needs.

    + Review of patient notes to help identify psychosocial needs and possible interventions/advocacy.

    + Build a rapport by providing continuity of care with patients and acting as their advocate throughout their hospital stay.

    + Collaborate between multiple care givers and foster communication between medical teams and nursing staff.

    + Participate in interdisciplinary care planning process with the Trauma Team.

    + Identifies and intervenes to coordinate care of trauma patients in relation to appropriateness of level of care, length of stay and resource utilization issues.

    + Identifying barriers to discharge and contacting relevant personnel to overcome these.

    + Initiates and monitors referral process of patients post discharge.

    + Demonstrates awareness of current trends that influence health care practice and assures compliance with professional, regulatory, governmental and organization standards.

    + Conduct screenings for trauma and substance use disorders among patients who have experienced traumatic injuries.

    + Provide brief interventions to patients who are identified as at-risk for substance use disorders or who have co-occurring trauma and substance use disorders

    + Participate in quality improvement initiatives that promote excellence in trauma care and SBIRT intervention

    + Building relationships with community partners to improve access to resources and services for patients who have experienced traumatic injuries.

    + Responds to screenings for Post-Traumatic Stress Disorder (PTSD) among patients who have experience traumatic injuries.

    + Providing education and resources to patients and families about PTSD, its symptoms, and available treatments with referral of patients to appropriate providers or resources

    + Responds to Level 1 activations to determine need for immediate intervention/family support

    + Works collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs. Maintains effective communication between members of the care team.

    + Identifies and addresses barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual.

    + Performs thorough assessments and develops appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.

    + Obtains input from the RN case manager to ensure the discharge plan is appropriate and aligns with the patient care plan. Reassesses and modifies patient discharge plan as needed, in conjunction with the clinical team.

    + Participates in the interdisciplinary care planning process and performs proactive discharge planning.

    + Provides education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage.

    + Coordinates and facilitates meetings between the care team and patients/families/caregivers. Assists patients and families in making difficult decisions while honoring their preferences and values.

    + Engages in community outreach and program development to support patients/families/caregivers.

    + Provides expertise in post-acute services and community resources.

    + Demonstrates knowledge of patient rights, advance directives, ethical issues, and confidentiality requirements.

    + Collaborates with the clinical care team to ensure the patients feel understood, heard and their perspective is valued.

    + Performs other duties as assigned.

    + Participates in Bio-Ethics committee and Baker Act Coalition.

    + Practices and adheres to the “Code of Conduct” and “Mission and Value Statement.”

    What qualifications you will need:

    + Bachelors Degree

    + Registered Social Worker (RSW)
