    Pay Rate: $25.00 per hour

     

    Hours: Monday-Friday 8:30AM-5:00PM (40hrs)

    OVERVIEW OF PRIMARY RESPONSIBILITES:

    The Community Supports Coordinator assists individuals with intellectual and developmental disabilities in achieving greater independence through the Community Habilitation Program. This includes providing direct support to help individuals develop, maintain, and strengthen essential life skills for independent living. The Coordinator also supports individuals in accessing, securing, and maintaining housing through programs such as the OPWDD Housing Subsidy and Integrated Supportive Housing (ISH). This is a primarily community-based role, which also involves case management, supervision, documentation, and report preparation.

    PRIMARY FUNCTIONS:

    + Housing Coordination:Support individuals in securing appropriate housing by navigating the OPWDD Housing Subsidy program, identifying suitable housing options, and assisting with the completion of required documentation. This also includes managing ISH set-aside units and collaborating with property management teams.

    + Waitlist Management:Identify, screen, and support individuals in the Community Habilitation program, as well as internal and external referrals, in maintaining a waitlist for those seeking independent living services.

    + Supervision:Responsible for supervising one program staff member.

    + Rep Payee Oversight and Independent Living Support:Provide oversight of the Representative Payee program and support Community Habilitation participants in developing and maintaining independent living skills.

    + OPWDD Housing Subsidy Budget Management:Conduct home visits and inspections, in addition, gather all necessary documentation for individuals participating in the program.

    + Collaboration: Collaborate with other service providers, care manager, to ensure coordinated care and support. Work as part of an interdisciplinary team to ensure the holistic needs of participants are met.

    + Crisis Intervention: Provide intervention and support in crises related to housing instability, Notices to Cure, and ensure immediate action is taken to address the issues.

    + Compliance:Ensure compliance with OPWDD guidelines and other applicable regulations and policies.

    ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:

    + Flexible hours may be required to meet the needs of individuals and families.

    QUALIFICATIONS AND ATTRIBUTES:

    Knowledge: Familiarity with the OPWDD services and knowledge of community resources and service providers is a plus:

    + Strong organizational, time-management, and multitasking abilities.

    + Excellent interpersonal and communication skills, with the ability to interact effectively with individuals, families, housing providers, and community partners.

    + Ability to work independently and as part of a collaborative team.

    + Strong problem-solving and crisis management skills.

    + Proficient in using Microsoft Office Suite and other case management software

    + Supervisory experience

    EDUCATION AND EXPERIENCE:

    Education: AA or BA preferred in Human Services Field.

     

    Experience: At least [3] years of experience working with individuals with intellectual disabilities, preferably in housing coordination, case management, or related fields.

    + Must be capable to sit or stand in front of a computer for long-periods of time

    + Able to work in open space floor plan

    + Must be capable to move throughout work day (and follow individuals throughout the community)

    + Work alongside co-workers within 3 feet

    + Must be able to move in tight spaces

    + Occasional lifting of > _25+pounds
