    Position Title : Patient Care Supervisor (Bilingual: Spanish)

     

    Reports To: VP of Home Care Operations

    Job Responsibilities:

    + Function as the liaison between coordinators and patients as needed

    + Provide direct case management services while maintaining accurate and verifiable documentation of all services rendered

    + Oversee implementation of policies and procedures that ensure the satisfaction of the patients serviced

    + Provide conflict resolution intervention and follow-up

    + Work closely with Team Leads and coordination team regularly to ensure quality of care is provided and continual progress on all case management goals are met

    + Support patients and families in an individualized basis by interpreting their needs and eliciting cooperation from other disciplines if required

    + Provide feedback from patients and families in order to improve the overall operations of the Coordination Department

    + Explain home health agency services and policies to patients and families

    + Participate in coordination team meetings, leading discussion around patients caseload

    + Provide report and progress documentation in a timely manner to allow for visibility into case status and productivity of the department

    + Provide consistent reporting on cases, discharges, patient feedback, to the Coordination Team Leads

    Job Requirements:

    + Experience using HHAeXchange and Salesforce is an advantage

    + Ability to show empathy and quickly build relationships with patients and families

    + Familiarity with Microsoft Office and mobile phone and web-based applications

    + Excellent verbal and written communication skills

    + Ability to maintain patient and caregiver confidentiality and privacy with dignity and respect

    + Critical thinker; ability to make decisions with intelligence, creativity, and kindness

    + Active listening skills

    + Excellent verbal communication skills both in person and on the phone\

    + Critical thinker; ability to make decisions with intelligence, creativity, and kindness

    + Ability to maintain patient and caregiver confidentiality and privacy with dignity and respect
