What You’ll Do
As the School Social Worker, you will provide client counseling, complete social service assessments and treatment plan maintenance. You will provide a range of education and planning for clients, families and staff. You will provide counseling to students and help them improve their academic achievement and social, emotional and behavioral competence. You will also help students with social skills, learning styles, mental health and behavioral issues.
Your Responsibilities Will Include:
+ Participating as a member of the admissions team.
+ Completing social service assessments.
+ Participating in the development and maintenance of the master problem list and social work treatment plans and identifying appropriate short and long-term social service goals for clients.
+ Participating in developing and carrying out behavior management programs as indicated.
+ Providing social work assessment, counseling, and referral as defined in the client's treatment plan.
+ Updating the master treatment plan based on the changes to the master problem list and social work treatment plans (not required for Social Worker -- Outreach).
+ Assisting client, family and team with discharge and long-term planning.
+ Participating in and providing recommendations for all relevant interdisciplinary team conferences.
+ Educating team members about specific psychosocial issues that may be impacting on client's treatment plan.
+ Establishing rapport with families and caregiver support systems with client consent and facilitating family meetings as needed.
+ Supporting and educating clients to relevant social, economic and mental health concerns to maximize independent living in individual and group settings.
+ Facilitating appropriate educational groups.
+ Carrying on a positive working relationship with both inter and intra Agency sources.
+ Maintaining professional ethics and adhering to the NASW code of ethics.
+ Maintaining documentation as defined in Agency's Policy Manual and as required by funding sources.
+ Continuing to develop professional skills by attending in-services and conferences.
You’re a great fit for this role if you have:
+ Master's Degree in Social Work, LMSW preferred (but not required).
+ Two+ years' experience providing social work services to children with intellectual and developmental disabilities (I/DD).
+ Genuine interest in working with the clients through their rehabilitative and psychosocial treatment process.
+ Ability to function as an integral member of a multi-disciplinary rehabilitation team.
+ Valid driver's license and reliable transportation.
Compensation
+ $65,000 - $80,000/year
