    JOB DESCRIPTION

    JOB TITLE: Case Manager

     

    PROGRAM: HRA Brooklyn SS/Vida Pathways

     

    FLSA STATUS: Exempt

     

    EMPLOYMENT STATUS: Full time,35 hrs

    JOB SUMMARY

    The Case Manager’s main responsibility is to provide case management services for a caseload of 22 clients and their family members. The Case Manager prepares comprehensive assessments and service plans. Home visits and Field work are required.

    ESSENTIAL FUNCTIONS AND KEY ACCOUNTABILITIES

    + Ensure that each client is receiving appropriate and effective case management services

    + Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals.

    + Conduct face to face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week.

    + Provide assistance in accessing primary medical care; assistance with adherence to medication regiments; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living.

    + Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients’ current needs.

    + Provide crisis intervention to clients and their families as needed.

    + Maintain ongoing awareness of clients’ medical conditions so that services are appropriate to changing needs.

    + Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services

    + Ensure that clients pay the balance of their rent, telephone and utility bills.

    + Provide counseling and orientation regarding tenant’s rights and responsibilities to maintain and retain housing.

    + Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and, provide transportation when necessary.

    + In collaboration with vocational counselor and entitlement specialist provide advocacy & referrals to employment programs for training and job placement in the private sector.

    + Assist clients in locating alternative housing options, when a client’s health so requires.

    + Provide and coordinate all supportive services as needed.

    + Collaborate with all providers within and outside of Comunilife involved with the client/family. Coordinate and collaborate with HASA personnel as required.

    + Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living

    + Maintain and complete required documentation including all contacts made with the client and any contact on their behalf

    + Participate in weekly supervision

    + Submit a weekly schedule of field visits

    + Participate in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors

    + Perform any other duty as assigned

    + REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

    + Extensive knowledge of case management principles and practices

    + Good initiative and learning ability of HRA-HASA Scattered Site Program Desk Guide and the HIV/AIDS Program, Office of Mental Health regulations.

    + Responsible, organized, effective, and efficient. Ability to perform multiple assignments simultaneously. Communication skills (effective & Actively listening, Verbal and written)

    + Community minded, patient, creative, flexible, compassionate, and culturally sensitive to persons who are diagnosed with HIV/AIDS, severe and persistent mental illness.

    + Ability to deal tactfully and diplomatically with other Comunilife employees, Federal, State, and City officials, professional and technical groups, clients, and the general public.

    Qualifications

    Education and Experience Requirement(s)

     

    Bachelors Degree required; with minimum one to two years field work experience HRA/HASA direct work experience. Associates degree with commensurate case management experience. Individual, family and group counseling experience required, particularly with HIV+/AIDS affected populations and/or substance abuse individuals. Preference given to persons with experience in Scattered Sites Programs, bi-lingual, bi-cultural in Spanish, a plus.

     

    Physical Demands

     

    Walking and climbing stairs.

     

    Working Conditions

     

    Home visits and field work required. General office work environment.

    + Conduct oneself in a manner which does not interrupt or interfere with and is conducive to the harmonious operation of the company’s business.
