Job Title: Clinical Specialist
DEPARTMENT: Home Care Cardiac
STATUS: Full Time Days
Summary: The Clinical Specialist, Home Care - Cardiac is responsible for the department, implementation, and evaluation of cardiopulmonary programs, and staff education in the care of cardiopulmonary clients within RRH Home Care. Responsible to develop, grow, monitor and continuously assess and improve the program striving to develop a nationally recognized program with excellent patient outcomes. Liaisons with other areas of RRH to ensure the highest quality of care and seamless transitions across settings. Provides in home direct care and consultations to high risk patients in area of specialty. Develops, mentors and educates a team of field clinicians that is highly competent to provide care to cardiac and pulmonary patients. Key Responsibilities: • Provides consultation to the Cardiac Nursing staff who is participating in the delivery of care to the high-risk cardiopulmonary clients, and to the generalist whom is caring for the less complicated Cardiopulmonary Patient. • Participates in the development and maintenance of current operational policies and procedures related to cardiopulmonary programs. • Participates in the selection of nursing specialty candidates for employment. Assumes major responsibility for development and implementation of orientation, education, and training activities for agency regarding cardiopulmonary program areas assigned. • Collaborates with the Quality Management staff to design, monitor, and evaluate the continuous quality improvement process, and risk management components of the clinical program. Conferences, joint visits with, and convenes specialty team on a monthly basis to evaluate and update program. • Identify and design research projects based on data generated from the continuous quality improvement program and clinical observations. • Educate the nurse generalists and health team members about technical, cognitive, and clinical developments emerging for the home care client. • Act as liaison with the medical community at large regarding Rochester Regional Home Care and Hospice’s Cardiopulmonary Nursing programs. • Perform direct care and documentation for select clients and caregivers that require specialist expertise and meets specified productivity. • Maintains current clinical knowledge in areas of Cardiopulmonary Nursing. • Participates on agency committees that are assigned. • Manage and evaluate the care deficit in complex cases. • Consult with staff and clients on ethical issues related to treatment and self-determinism. • Participates in the development of a multidisciplinary and inter-organizational plan of service in order to provide appropriate Cardiac patient assignment to the Cardiac. • Consults with Supervisory staff in order to provide appropriate Cardiac patient assignment to the Cardiac team. • Consistently demonstrates high standards of integrity by supporting Rochester Regional Home Care and Hospice’s mission and values and adhering to the Corporate Code of Conduct. • Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures. • Regular and reliable attendance is required Desired Attributes • BSN preferred. • Home Health nursing experience preferred. • Expertise and advanced knowledge in cardiac care, practices and procedures preferred. • Demonstrated success in program development, including the development and implementation of evidence based practices and best practice guidelines, goal setting and outcome achievement preferred. • Proven effectiveness in mentoring and educating others Home Care experience preferred. • Strong interpersonal, organizational, and time management skills. Minimum Qualifications: • Graduate of an accredited school of nursing • At least 2 years nursing experience in cardiac care • Must have ability to travel. Required Licensure: • Valid New York State RN Nursing license required.

AS: Nursing (Required)
LICENSES / CERTIFICATIONS:
BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

$71,000.00 - $95,000.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
