    Job Summary:

    This pharmacist will operate under our ambulatory care pharmacy services umbrella and primarily provide direct patient care within our ambulatory cardiology clinic. This pharmacist will work directly with providers and support staff to optimize medication therapy, provide patient/provider/nurse education, and assist with transitions of care management. This pharmacist will also work closely with our specialty/retail pharmacy division to provide medication oversight and monitoring in accordance with specialty medication requirements. As a member of a diverse ambulatory care pharmacy team, this pharmacist may also be called upon collaborate in the provision of patient care to numerous Upstate Clinics. These clinics may include adult medicine, family medicine, rheumatology, pulmonary, international health, cystic fibrosis, endocrinology, and neurology.

     

    As a member of the broader pharmacy department and depending upon department need, job duties for the position could include but are not limited to: Interpretation of medication orders and prescriptions to ensure they are complete, safe, rational, and compatible with institutional policy. Pharmacists have responsibility for preparing compounds and dispensing medications for inpatient and ambulatory care use. This includes the preparation of injectable sterile products and non-sterile compounds. Pharmacists may be responsible for performing duties in various practice locations, including inpatient, emergency departments, ambulatory care clinics, and transitions of care settings. Pharmacists at Upstate are expected to provide information to health care practitioners to promote the safe and rational use of medications. Furthermore, pharmacists shall provide drug monitoring services, maintain pharmacy records according to law and institutional policy, interpret policy and procedures for hospital staff, participate in teaching programs for the department and hospital staff, and participate in the department’s quality assurance programs.

    Minimum Qualifications:

    Preferred Qualifications:

    PGY-2 Residency Training in Ambulatory Care or Cardiology; Board Certification, eligibility to engage in Collaborative Drug Therapy Management (CDTM) according to NY State Requirements and/or previous ambulatory care pharmacy practice preferred.

    Work Days:

    Predominantly weekdays (possibility for evenings/weekends/holidays exist based upon department need)
