    Educational Requirements: Baccalaureate or master’s degree

     

    Experience Required: Minimum of 1-year related experience.

    Essential Responsibilities

    Resident Case Management:

    1. Initial and periodic evaluation, at least once every 12 months, of the needs of the resident and the capability of the facility program to meet those needs.

    2. Orienting a new resident and family to the daily routine.

    3. Assisting each resident to maintain family and community toes and to develop new ones.

    4. Encouraging resident participation in facility and community activities.

    5. Establishing linkage with and arranging for services from public and private sources for income, health, mental health, and social services.

    6. Assisting residents in making application for, and maintaining, income entitlements and public benefits.

    7. Assisting the resident in obtaining and maintaining a primary physician or source of medical care choice, who is responsible for the overall management of the individual's health and mental needs.

    8. Assisting the resident in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the resident's health or mental health including but not limited to:

     

    A) Health and mental health services

     

    B) Dental Services

     

    C) Medication

     

    D) PT/OT Services

     

    9. Assisting residents in need of alternative living arrangements to make and execute sound discharge or transfer plans.

    10. Assisting residents in grievances and recommendations.

    11. Actively participate in quality assurance meetings.

    12. Provide input and assistance in creating and implementing facility policies.

    13. Provide input and assistance in facilitating staff in-services and education.

    Admissions and Marketing:

    1. Complete initial request for information.

    2. Schedule and complete tours.

    3. Provide assistance with pre-admission evaluations and review required medical evaluations.

    4. Complete and review all required admission paperwork.

    5. Upload required documents to the electronic medical record.

    Leadership and Supervision of Staff:

    1. Exercises leadership style that fosters collaboration and achieves consensus and cooperation.

    2. Obtains input from staff via regular formal and informal meetings

    3. Takes timely and appropriate actions to resolve identified issues

    4. Resolves conflicts as they may arise

    5. Initiates change to improve the quality of care within the facility

    Documentation:

    1. Reviews pre-admission documentation for compliance.

    2. Maintains clinical records on all residents. Documents actions in an appropriate and timely manner.

    3. Develops tracking system to assure the timely completion of periodic assessments.

    4. Maintains compliance with NYS/DOH regulations.
