    Empowering People's Independence (EPI), provides support for people with developmental disabilities, epilepsy, and brain injury through progressive, tailored care to improve their health, wellbeing, independence, and social inclusion.

    Job Purpose:

    The primary purpose of this role is to execute Empowering People’s Independence (EPI) strategic objectives to include, but no limited to; coordination of intake services, processing of applicants and facilitate service events.

    Essential Functions:

    + Ensure individuals are identified meet both eligibility criteria and funding source by thoroughly screening & processing applications in a timely manner.

    + Respond to phone calls & emails regarding Epilepsy Services requests.

    + Attend and facilitate EPI Services events and support group(s) as required.

    + Field all phone calls from individuals, family members, Care Managers/ Care Coordinators, etc. regarding services provided by EPI, to include service-related information from the community at large.

    + Provides referral source with requested information related to EPI.

    + Sends application packet to referral agent for completion, informs of all attachments to the packet including check list of items that will need to be sent in with application packet and scans completed documentation to program manager/supervisor/designee.

    + Complete residential intake process when openings occur at EPI Individualized Residential Alternatives (IRA).

    + Chair Intake Committee meetings to include and maintain annual calendar of meetings, ensure all information is scanned into intake folder for committee review, issue approval/ denial letters.

    + Respond to all intake inquiries.

    + Ensure all information regarding Epilepsy Services grant monies are tracked and reports completed.

    + Attends external agency fairs, etc., as assigned.

    + Oversee Life Plan emails to include tracking when Life Plans are due, ensuring distribution to departments/ QIDP team.

    + Send out Self- Direction utilization reports monthly.

    + Report all policy violations to appropriate personnel.

    + Promote workplace safety.

    + Complete other duties as assigned within scope of practice by Intake Liaison Manager, Department Director and/or designee.

    Non-Essential Functions:

    + Participate on various committees as able.

    Compliance:

    + Must maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to board/vendors/donors’ names, investigations, finance, operations, and employee relations matters.

    + Must complete specific company required trainings as mandated by our governing agency, Office for People with Developmental Disabilities (OPWDD), at time of hire and annually thereafter.

    Adhere to all company/departmental policies & procedures and ensure departmental compliance.

    Education and Experience:

    + Minimum GED with 3 years’ relative experience in field; or any combination of post high school education and 2 years relative experience. Knowledge and experience in OPWDD/DOH field preferred.

    Supervision:

    None

    Computer Skills:

    + Strong working knowledge of Microsoft programs to include Email, Word, Excel, PowerPoint, Teams. Experience with electronic records systems.

    Qualifications:

    + Must possess and maintain a valid NYS Driver’s License- a vehicle is required for business travel.

    + Mature interpersonal/professional skills to effectively collaborate, resolve issues and maintain confidentiality.

    + Good verbal and written communication skills.

    + Strong numerical comprehension.

    + Demonstrated ability to use poise, tact, and diplomacy, during difficult and/or stressful situations.

    + Proven ability to work in a fast-paced environment, with demonstrated ability to manage multiple tasks, and customer requests, while meeting deadlines.

    + Must be able to talk, hear, stand, walk, and/or sit for extended periods of time; may have to twist, bend, use hand to finger, stretch with hands and arms.

    + Specific vision abilities required by this job include close vision and ability to adjust focus

    + May be exposed to strong odors from time to time.

    + Must be able to lift and carry up to 50 lbs.

     

    While performing the duties of this position, exposure to changing weather conditions will occur.
