The role** **you** **’ll** **contribute** **:

The Registered Professional Nurse (RN) Resource is responsible for the delivery of coordinated nursing care for a patient and/or assigned group of patients within a patient care unit and for the day-to-day management and supervision of related clinical activities to which assigned. The RN Resource uses the nursing skills of assessment, planning, implementation, and evaluation in their patient care delivery. The RN Resource practices within State, facility, and regulatory guidelines. The primary concentration of the RN Resource focuses on the strategic needs or priorities of a nursing department specific to patient admission, discharge, or duties as assigned are directly related to patient and facility outcomes as delegated by the Department Manager or Director.

+ Demonstrates effective assessment skills to ensure quality patient care is provided.

+ Performs and documents accurate admission assessment per unit standard which may include initial observation, obtaining and interpreting information in terms of the patients' needs, safety, discharge planning, psych/social/cultural, family needs, multi-system physical assessment, patient/family level of knowledge and readiness to learn about disease process and care requirements. Reports appropriate information in a timely manner to the primary nurse assigned or the unit clinical leader.

+ Initiates core measure and zone tool requirements during the admission process on relevant patients with subsequent handoff communication to the primary RN.

+ Utilizes clinical data to implement appropriate interventions during admission and discharge processes in coordination with the primary nurse or the unit clinical leader.

+ Works with the primary RN to develop, prioritize, and document a plan of care that is individualized and involves patient, family, and/or significant other in goal setting per unit standards.

**Minimum qualifications** :

+ Graduate of an accredited school of nursing, minimum of an associates degree as a Registered Nurse Required
