Case Manager

Volunteers of America Delaware Valley is seeking a Case Manager to join our treatment team within the Community Housing and Supportive Services and Behavioral Health and Reentry Services divisions. The Case Manager plays a key role in supporting individuals in achieving stability, independence, and long-term success by developing client-centered service plans and linking individuals to essential community resources.
This is a direct-service role ideal for compassionate professionals with strong organizational skills, experience in human services, and a desire to support special populations, including justice-involved individuals and the unhoused.
What You’ll Do:
+ Conduct initial client intake, orientation, and risk/needs assessments using approved agency tools
+ Develop, implement, and regularly update individualized service plans
+ Monitor client progress and ensure compliance with treatment, housing, and programmatic goals
+ Facilitate regular case management sessions and group programs addressing key life domains
+ Serve as a liaison between program, funding sources, and community organizations
+ Link clients to essential services, including medical care, education, employment, and housing
+ Prepare all required documentation and reports in compliance with agency and funder standards
+ Support discharge planning to ensure continuity of care
+ Participate in treatment team meetings and contribute to collaborative decision-making
+ Conduct recreational coordination, medication monitoring, and advocacy as required by site
+ Attend court hearings or act as a liaison as assigned
+ Addendum-Specific Responsibilities (Based on Location): Agape House, Amani House, Aletha R. Wright, Anna Sample, Eleanor Corbett House: Focus on community liaison, client advocacy, and recreational access
+ Station House: Emphasis on financial skills education, health advocacy, credit building, housing liaison work
+ ISSI and Step Down: Ensure medical needs and medication compliance are consistently managed
What You Need:
+ Bachelor’s Degree in Human Services or a related field preferred; transferable experience will be considered
+ Experience with underserved or special needs populations is highly valued
+ Excellent verbal and written communication skills
+ Strong ability to manage documentation and meet deadlines
+ Basic computer literacy
+ Valid driver’s license and ability to meet agency driver standards
+ Bilingual abilities are a plus
+ Must be able to pass criminal background check
