Prestige Care Center of Fairfield is seeking a FT Social Services/Admissions Director. Long-term care & skilled nursing facility experience preferred. Knowledge of EMR, completion of resident assesment and designated areas of resident MDS, Social history, discharge planning, progress notes, and PASRR required. Skills to network with community agencies and resources, hospitals and clinics. Coordination of admission referrals and effective communication with the interdisciplinary team. Experience with Medicare & Medicaid payment sources.
