    Position Summary

    Trinity Health Alliance of Michigan (THAM) is seeking a dynamic and highly engaged **Practice Coach - Office Specialist** to join our team. This individual will serve a critical role in supporting physician practices affiliated with THAM by coaching and guiding process improvements that enhance clinical outcomes, patient experience, and operational efficiency.

     

    The Practice Coach works directly with physicians and office staff to implement data-driven strategies that support value-based care contracts and achievement of the **Quadruple Aim** . This position plays a vital role in advancing THAM’s mission and performance by fostering strong relationships and offering tailored, hands-on support to practices across our network.

    Key Responsibilities

    + Coach physician offices to enhance performance on clinical and operational metrics

    + Facilitate quality improvement efforts related to clinical integration, total cost of care, and credentialing

    + Present performance data and collaborate on actionable improvement plans

    + Train and support practice staff on systems, protocols, and best practices

    + Coordinate with THAM teams and leadership to support provider success

    + Maintain accurate tracking, monitoring tools, and engagement documentation

    + Serve as a liaison between practices and payors when necessary

    + Educate practices on THAM tools, resources, and quality reporting systems

    + Support onboarding and education for new providers and staff

    Qualifications

    Education:

    + **Required:** Certified Medical Assistant or Associate’s Degree with a minimum of **5 years of progressive healthcare experience**

    + **Preferred:** Bachelor’s degree in Healthcare Administration or related field

    + Experience with practice coaching, clinical integration, and/or Patient-Centered Medical Home models is strongly preferred

    Skills & Competencies:

    + Exceptional interpersonal, coaching, and communication skills

    + Ability to analyze data and develop actionable insights

    + Strong organizational and time management skills

    + Proficiency with Microsoft Office Suite (especially PowerPoint and Excel)

    + Ability to travel locally to provider sites (at least 50%)

    + Experience with Population Health tools and CRM systems is a plus

    + Demonstrated initiative and adaptability in fast-paced environments

    Working Conditions

    + Fast-paced environment with frequent local travel

    + Requires balancing multiple projects and adapting to shifting priorities
