    Description:

    SUMMARY

    This position is responsible for providing Central Valley Health Plan with extracting or obtaining data related to patient safety and medication adherence in order to meet HEDIS goals set by CMS.

    REQUIREMENTS

    1. High school diploma or equivalent is required.

    2. California State Board of Pharmacy registration as Pharmacy Technician is required.

    3. Ability to interact with a computer system and software such as Microsoft Excel or Word is required.

    4. Ability to work as part of a collaborative team is required.

    5. Ability to demonstrate strong attention to detail and accuracy is required.

    6. CPhT certification is preferred.

    ROLE SPECIFIC ACCOUNTABILITIES

    1. Demonstrate proficiency in understanding HEDIS measures as defined by CMS.

    2. Extract data from electronic medical records to submit to CMS.

    3. Make calls to members or providers to ensure medications are taken timely in accordance with the patient safety adherence measures.

    4. Download Reports from Health Plan platforms. Trend and perform analysis of pharmacy utilization.

    5. Send faxes to providers to ensure medications are being sent to the pharmacy in a timely manner to improve compliance with the patient safety adherence measures.

    6. Electronically transmit prescriptions designated by the clinical pharmacists for day supply as outlined by the standard operating protocols with Health Plans.

    7. Assist members with enrollment in the Health Plan pharmacy and ensure providers have updated pharmacy information.

    8. Review and filter reports for Pharmacist review such as the adherence and statin reports.

    9. Obtain medical records for medication reviews completed at provider’s office to meet the Care of Older Adults: Medication Review measure. Assist with clinical pharmacist scheduling to complete medication reviews for members whose providers have not completed one.

    10. Assist with scheduling and/or preparation of medication reviews by pharmacist after hospital discharge.

    11. Maintain records of activities completed including month to month or year to year comparisons to show productivity and improvement.

    12. Communicate with team members in daily and/or weekly discussions to help monitor progress and improve HEDIS measure productivity.

     

    Pay Range: $21.98 - $30.77
