POSITION STANDARDS

Minimum Qualifications:
* Education: Master's or Doctoral degree in Psychology, Social Work, Counseling, or a related behavioral health field.
* Experience: At least 3 years of post-licensure clinical experience in behavioral health, with a minimum of 1 year in a supervisory or leadership role.
* Specialized Knowledge: Experience with integrated care models, embedded behavioral health, or working in primary care settings is preferred.
* Ability to work effectively in a collaborative, multidisciplinary team
* Excellent verbal and written communication
* Ability to maintain a flexible work schedule, including occasional evening or weekend hours asRequired Licensure/Certification Skills:
* Licensure: Current, valid licensure (e.g., LCMHC, LICSW, LMFT, PhD, PsyD) in the state of New Hampshire and the State of Rostered Clinical Supervisor with the State of New Hampshire, or able to become rostered within 1 month of hire.

PART TWO:FUNCTIONAL RESPONSIBILITY
*
Position Objective
*
The Clinical Supervisor will provide oversight, guidance, and clinical supervision to a team of behavioral health professionals, ensuring high-quality services and outcomes for primary care patients served. This position requires a collaborative approach with multidisciplinary teams, including primary care providers, consulting psychiatrists, and other specialists, to support CoCM (Collaborative Care) within Primary Care.

Performance Expectation

Clinical Supervision :
* Provide regular supervision and support to behavioral health clinicians, student interns, and resource specialists to ensure they adhere to best practices, ethical standards, and clinical protocols.
* Provide clinical oversight and supervision, as required by The State of New Hampshire to unlicensed clinicians.
* Assist in complex case management by providing expert consultation and oversight of patient cases, addressing treatment planning, risk assessment, and crisis management.
* Facilitate and support professional development opportunities for the behavioral health team.
* Oversee and support accurate and timely clinical documentation, including treatment plans, progress notes, CoCM documentation and other required records. Ensure compliance with billing, coding, and other administrative requirements.
Program Integration, Management and Development:
* Work closely with primary care providers, leadership and community providers to support the implementation of CoCM programming, ensuring quality and consistency in care delivery.
* Establish and maintain evidence based clinical programs, evaluating their effectiveness and recommending improvements to the Practice Director.
* Coordinate consulting psychiatry provider schedules and ensure patient access to embedded specialty care consultation.
* Support implementation of substance use programming, providing clinical program oversight and support to the team as needed.
Quality Assurance:
* Monitor and evaluate the effectiveness of behavioral health interventions and Ensure compliance with internal policies, state/federal regulations, and accreditation standards.
* Regularly review CoCM standards and guidelines to ensure quality assurance
Cultural Competence & Sensitivity:
* Support the delivery of culturally sensitive care to diverse populations, promoting an environment of inclusion, respect, and awareness of cultural differences in behavioral health needs.
Crisis Intervention:
* Provide guidance and support during critical situations, including crisis management, safety planning, and coordination with emergency services when necessary.
Patient Care (Behavioral Health Care Manager) :
* Screen and assess patients in primary care for common mental health and substance use disorders and provide patient education, as well as available treatment options.
* Provide brief behavioral interventions using evidence-based techniques, such as behavioral activation, problem-solving treatment, motivational interviewing or other treatments as appropriate.
* Systematically track patient follow up, treatment responses, and clinical outcomes using the CoCM registry.
* Support psychotropic medication management for primary care patients, as prescribed by medical providers, focusing on monitoring treatment adherence, side effects and effectiveness of
* Participate in regularly scheduled caseload consultation with psychiatric consultants, with an emphasis on reviewing patients who are new to the caseload and those who are not improving as expected under the current treatment plan.
* Facilitate treatment plan changes, in conjunction with the patient’s primary care provider and assigned psychiatric consultant, for patients who are not improving as expected and who may need more intensive and more specialized mental health care.
* Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
