    Social Workers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs that promotes continuity of care, and facilitates transitions to the next level of care. The social worker will provide education, consultation, and serve as a liaison to the healthcare team regarding the patient and families' emotional, psychological, social, financial and developmental aspects of care.

     

    The Palliative Care Social Worker will work with the Palliative Care Team which is a multidisciplinary team including physicians, nurse practioners, social work, and chaplains. The Palliative Care Social Work will need to be competent in integrating grief theories into practice and family-team conferencing. Palliative Care Social Workers will also advocate for holding focus on palliative and end of life care on patient choices, preferences, values and beliefs. The Palliative Care Social worker will demonstrate awareness of compassion fatigue and the ethical responsibility to mitigate the condition.

     

    The Palliative Care Social Worker will also coordinate discharges to home, inpatient hospice facilities, and to nursing facilities.
