LPN On Call Hospice

Responsible for participating as a professional member of the hospice interdisciplinary team by providing skilled nursing assessment and hospice care in order to maximize the comfort of patients and families. Care is consistent with Scotland Regional Hospice (SRH) policies and procedures. Managing the immediate care needs of the entire home care caseload during non-office hours and is expected to cover Morrison Manor the Inpatient facility if needed
Reports to the Director of Clinical Services and is under the direction and leadership of the Administrator on call. Supervises hospice aides performing personal care services after hours.
JOB DUTIES AND STANDARDS
Together with the RN ON Call, the On-Call LPN is responsible for
1. Assumes responsibility for evening and weekend on-call duty as scheduled and consistent with SRH’s policies and procedures. Duties may include triaging calls, making routine and urgent visits, presentations and admissions and attending deaths. The afterhours nurse may be responsible for patient care in any setting, home, nursing home, assisted living, hospital or The Morrison Manor.
2. Assumes primary responsibility for SRH’s total care census including assessing, planning, implementing and evaluating phases of the nursing process as appropriate. Is proficient in providing symptom management and comfort care.
3. Performs late-day admissions and scheduled patient visits as assigned by the Director of Clinical Services or Administrator on call.
4. Responds to medical or emotional crisis and/or death of patient by making home visits to provide direct care as appropriate. Provides counseling at time of death.
5. Initiates communication with attending physicians, medical director, and other IDT members as needed to coordinate optimal care for the patient/family.
6. Maintains up to date patient documentation so that problems, plans, interventions and goals are accurate and changes are reflected as they occur.
7. Participates in the ongoing teaching and supportive care goals by instructing primary caregivers and family members.
8. Responsible for providing Morrison Manor nurses with consultative services, performing admissions, and providing staffing during periods of crisis
. 9. Demonstrates familiarity with SRH policies and rules; state, federal and accreditation regulations as they relate to the delivery of hospice care.
10. The LPN On-Call is expected to cover Morrison Manor the Inpatient facility a minimum of 2-4 hours every Saturday and Sunday mornings, make scheduled weekend visits as needed, and triage incoming calls/visits with the On-call RN as needed. When the On-Call RN is pulled to cover a shift at Morrison Manor, the On-Call LPN will be responsible for triaging all calls and visits while utilizing the support staff on call as needed.
10. Serves as the primary nurse for Morrison Manor during staffing shortages as directed by the Director of Clinical Services or Administrator on-call.
11. Other duties as assigned by the Director of Clinical Services or Administrator on call
QUALIFICATIONS
EDUCATION
Minimum Requirements: North Carolina or multi-state LPN License with two years of LPN experience.
