    Position Summary:

    The PRN Pharmacist provides intermittent, as-needed coverage to support pharmacy operations in a Durable Medical Equipment (DME) setting. This role steps in during staff absences, peak workload periods, or other coverage needs. Responsibilities include verifying and processing prescription orders, ensuring regulatory compliance, supporting patient care through medication consultation, and maintaining accurate documentation. The PRN Pharmacist works collaboratively with the care team to ensure consistent service and adherence to company protocols and industry standards.

    Key Responsibilities:

    + Review, receive, and verify all new patient orders and documentation.

    + Collaborate with physicians and healthcare professionals to ensure safe and effective medication use.

    + Provide consultations and respond to inquiries from patients, caregivers, and healthcare providers regarding medications and their appropriate use.

    + Counsel patients and caregivers on specific drug therapies, including possible adverse effects and drug interactions.

    + Monitor and assess ongoing drug therapy for:

    + Therapeutic appropriateness of medication(s)

    + Duplication within the patient’s medication regimen

    + Appropriate dosage, frequency, and route of administration

    + Adherence, potential interactions (drug/food/test), and disease-related limitations.

    + Maintain complete, accurate documentation of all clinical activities and communications in the patient record.

    + Order, inventory, and maintain medication supplies.

    + Prepare, label, package, and dispense medication orders in accordance with standard operating procedures.

    Qualifications & Responsibilities:

    Required:

    + Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy

    + Current, active pharmacist license in good standing in [Insert State]

    + Strong clinical knowledge and understanding of pharmacotherapy principles

    + Excellent communication, documentation, and organizational skills

    + Proficiency in pharmacy systems and electronic medical records

    Preferred:

    + Previous experience in home healthcare or specialty pharmacy

    + Familiarity with inventory management systems
