    Reports to: Residential Manager

     

    Job Classification: Full-Time

     

    FLSA: Non-exempt

     

    Operation hours/Days: 8 hours Shift (Monday - Friday)

     

    Range Salary: $20.00 - $21.00 per hour

    Brief Job description: Medical Coordinator ensure that the residences assigned is operating effectively and that they are in compliance with the policies and procedures set by the Agency, OPWDD and other relevant State, Local and Federal agencies.

    Essential Functions

    + Ensure all appointments are on the calendar in the supervisor's office and create a paper calendar to be sent nurse, residential manager, supervisor, and BIS. Remember to update any appointments that are added or changed during the month.

    + If the provider cancels an appointment, notify the team via email.

    + Any medication changes must be brought to the nurse's attention on the day the change is ordered.

    + Follow up with pharmacy to ensure delivery of all new medications with the accompanying scripts and formulary.

    + Medication inventory to be completed every Monday, reorder any medications that are needed, obtain scripts for any medications that have no refills

    + Communicate with management whenever extra staff is needed for 1:1 coverage

    + Obtain MD consults & ensure they are filled out correctly & entirely at the time of the appointment

    + Seizure/helmet protocols should be obtained during respective appointments – scripts for helmets need to be renewed yearly by the discipline that wrote the previous script, I.E., if Neurology, then obtain a new script and protocol from Neuro; if psych, then obtain a new script and protocol from psych

    + Seizure protocols need to be brought to Neuro at least yearly for review.

    + Review all medical consults before leaving the medical appointment, if possible—if unable, then must review before sending out to team—if something does not look right or does not match what the M.D. told you, then get it clarified.

    + If a behavior should occur during an appointment, an ABC sheet must be filled out—it gets scanned with your team emails, and the original gets given to the individual's residence hab; indicate who it was given to in the team email.

    + Responsible for reviewing and updating medical binders to schedule appointments in a timely fashion

    + Ensure that all medical appointments are made within the time frame given to ensure compliance, however, own your calendar—do not accept appointment(s) if you already have one scheduled for that date and time.

    + Must be able to transport individuals to & from their medical appointment.

    + When scheduling appointments - No overlapping times; ensure enough time between each appointment to review the medical documentation. The suggestion is to run one appt in the am and one in the afternoon.

    + Ensure that all consults, test results, labs, etc. are placed in the incoming binder for the house nurse to review after each appointment

    + Request all lab or test results from M.D. timely (results should be in the incoming binder within one week): USE THE PATIENT PORTAL.

    + Schedule & complete all specialty tests ordered by MD within 14 days; if unable to, then it cannot be later than 30 days, unless otherwise specified by M.D., such as X-rays, MRIs, lab work, etc.

    + Pharmacy Reviews should be taken to each consumer's Primary MD to be signed in a timely fashion; they need to be signed and picked up within two weeks. If you are having difficulty getting them back, let the nurse know so they can assist you in obtaining them.

    + Obtain a copy of the e-script from the medical providers whenever able, and ensure that all scripts in the MAR are up to date.

    + Notifications must be posted on the medication cabinet for any changes in medication and remain up for one week.

    + Verbal orders from Primary MD need to be followed by an actual script

    + Ensure that all OTC sheets are up to date in the residence.

    + Team/D.H. emails should be sent after each appointment - Proofread each email for grammar & spelling; include the supervisors on the team email. Include medical/psych consult and all scripts, headcount sheets, consents, and ABC sheets if applicable. This should all be in one email.

    + If consent is needed to be signed by a parent or ICC, the original is to be given to the management team; indicate in the team email to which it was given; if no management is available, then put in the incoming binder; however, this would need to be communicated in the team email

    + Scheduled PTO: do not schedule any appointments for the days you are on scheduled PTO unless necessary and you have spoken to the Asst. Manager/Manager to ensure the residence can run it/them.

    + Other miscellaneous tasks - Subject to change as situations arise or as directed by your supervisor or nurse.

    Education & Experience:

    + Bachelors or 2 years experience in the field.

    + OPWDD: 1 year

    + Driver's license
