**The Veteran Health Navigator Supervisor will oversee the work of the Veteran Health Navigator team who are the p** oint of contact for clinical providers to refer Service Members, Veterans and their families (SMVF) to the Veteran Health Navigator Program to connect with VA health benefits, resources, and state Veteran resources as appropriate. The Veteran Health Navigator Supervisor will lead the efforts with the Veteran Health Navigator team with community outreach, integration efforts with non-VA healthcare systems, and overseeing Veteran Health Navigators.
Minimum Qualifications
+ Current Licensure in Minnesota as a Licensed Social Worker (LSW), or higher* or ability to obtain licensure by date of hire.
AND
+ Two (2) years of experience in a healthcare or social services setting.
+ Experience as a supervisor, team lead, lead worker, or project manager.
+ Knowledge of healthcare regulations and the ability to advise on practices with screenings, provide program evaluation, and training and education.
* Licensed Graduate Social Worker (LGSW), Licensed Independent Social Worker (LISW), or Licensed Independent Clinical Social Worker (LICSW)
Preferred Qualifications
+ Experience adhering to healthcare regulations as set by Centers for Medicare and Medicaid Services (CMS), or other regulatory agencies such as Det Norske Veritas (DNV) or Joint Commission.
+ Experience working with Veterans programs and services.
+ Experience working with either the Veteran and/or active-duty service member population.
+ Experience in one of the three areas:
+ Case Management
+ Data Collection and Analysis
+ Continuous Improvement
+ Community Outreach

This position requires successful completion of the following:
