Job Descriptions:

New England Cancer Specialists (NECS) is currently seeking a full-time 40 hrs a week Survivorship/High Risk Health Coach and Wellness Nurse. The High Risk & Survivorship Nurse Navigator and Wellness Health Coach at New England Cancer Specialists is responsible for providing personalized health coaching and supportive care coordination for cancer survivors and high-risk patients. This role plays a key part in patient education, follow-up, and engagement, ensuring a holistic and informed approach to long-term wellness. This position is an essential role in the Survivorship and High-Risk Cancer Genetics Clinic.

Overall Duties and Responsibilities:

+ Provide one-on-one health coaching sessions tailored for survivorship patients.

+ Serve as the Wellness Program Coordinator, which includes:

+ Educating patients about NECS wellness programming.

+ Enrolling eligible participants when appropriate

+ Conducting pre- and post-assessments on site with support of Clinical Medical Assistant.

+ Providing on-site support during meetups.

+ Work with NECS genetic counselors and High-Risk Providers to ensure that high-risk patients have appropriate follow-up care.

+ Assess and triage the urgency of referrals for Survivorship and High Risk. Contact referrals with an introduction to the services provided and schedule in a clinically appropriate time frame.

+ Distribute educational materials to patients as directed by providers via phone, CareSpace messages, or patient portal uploads.

Required Experience:

+ BSN Required

+ Active Maine and New Hampshire RN license required.

+ Two years plus nursing experience required.

+ Nurse Health Coach (NC-BC) required or the ability to obtain with in 1-2 years of hire when eligible based on the American Nurses Holistic Association.

+ Hematology/Oncology experience preferred.
