Reentry Care Coordinator

Under the supervision of the Director of Treatment Services, the Reentry Care Coordinator shall be responsible for the coordination of services for individuals in the Douglas County Community Corrections Facility. The Reentry Care Coordinator shall operate in an ethical, professional manner, and interact with others in such a fashion as to create and maintain respect for the agency. They shall act in accordance with the policies determined by the Board of Directors and shall abide by all applicable federal, state and local regulations. Specifically, those related to Social and Rehabilitation Services/ Commission on Substance Abuse, Mental Health and Developmental Disabilities/Substance Abuse Treatment and Recovery Licensure Standards and any contractual agreements. The duties of this position include referral services, educational presentations, one-on-one meetings with clients, non-clinical group facilitation, and all related paperwork demands.
Duties and Responsibilities:
1. Abide by policies related to the safety and security of the facility.
2. Continually develop knowledge, skills, abilities, and personal characteristics needed to function within the correctional environment, such as Interpersonal Communication Skills, Motivational Interviewing, and Effective Practices in Correctional Settings II (EPICS II)
3. Develop collaborative working relationships with community partners such as District Attorney’s Office, District Court and Municipal Courts in Douglas County, Pre-Trial Supervision, etc.
4. Be committed to the continual improvements of the day-to-day operations of the Programs and Reentry Division
5. Ability to effectively and efficiently utilize the facility’s information with technology
6. Conducting SUD screenings of inmates identified by Douglas County Jail through the booking assessment of those identified by Douglas County Community Corrections Facility (DCCCF) as being at risk.
7. Providing education, community resource information, coordination of SUD assessments through WAC, referral to Mirror treatment & MAT programs, and other community providers as need is identified.
8. Collaboration with Douglas County Jail Reentry team and other community partners through a multidisciplinary approach.
9. Facilitation of non-clinical SUD programming groups (SMART, Recovery Support Groups, etc.) and activities as scheduled by Douglas County Reentry Team.
10. One-on-one meetings with inmates to coordinate care upon release from custody of DCCCF.
11. Data collection in accordance with Mirror outreach data collection in Credible.
12. Participate in supervision and training as identified and required by Mirror.
13. Other duties as assigned
Qualifications
1. Graduate of a four-year accredited college or university with a bachelor's degree in social work, sociology, psychology, substance abuse counseling or a closely related field is preferred. Master's degree matching one of the aforementioned professions would be desirable.
2. Minimum of one-year paid experience providing case management.
3. Although not mandatory applicants who are bilingual (English & Spanish) will be given strong consideration.
4. Excellent interpersonal skills are required, as well as the ability to communicate effectively both verbally and in writing.
5. Capable of maintaining a team spirit and the ability to work cooperatively with others.
6. Must be adaptable to change and have effective organizational skills.
7. Must be able to organize and follow through on multiple tasks.
8. Display consistent, good judgment and decision-making skills.
9. Ability to establish and maintain rapport with target population.
10. Must be able to pass background/security check including a Sheriff’s Office background investigation and polygraph.
11. Employees with a diagnosed substance use disorder or behavioral addiction must demonstrate a minimum of two consecutive years in sustained recovery, including
abstinence from all intoxicating substances and the absence of active symptoms. They must also be in full, sustained remission.
12. Must be proficient in Microsoft Office Word, Outlook, and with a basic understanding of Excel preferred.
13. Position may also involve bending, walking stairs and lifting up to 40 pounds on occasion.
