Job Description:

A Brief OverviewCoordinates data collection for the Resident Assessment Instrument (RAI), including the MDS, CAAS and the Multidisciplinary Care Plan for long term care.What you will do

+ Completion of the MDS, CAAS and Care Plans are done by required completion dates and submit to the state of Iowa by submission date.

+ Evaluate nursing and clinical intervention outcomes, conferring with other health care team members as necessary.

+ Monitor resident medication usage and results.

+ Participates in discharge planning and other necessary team conferences.

+ Interview resident as designated and review documentation related to medical conditions, such as adverse reactions to medication or treatment, condition change, or progress or lack of progress with interventions.

+ Work as part of a health care team to assess resident needs, plan and modify care, and implement interventions as appropriate.

+ Demonstrates accurate keyboarding proficiency and computer navigation skills.

+ Participate in activities aimed at professional growth and development including conferences or continuing education activities.

+ Collaborates with assigned RN to refer residents requiring more specialized or complex treatment to psychiatrists, primary care physicians, or other medical specialists.

+ Must have current knowledge of the entire RAI process and current regulatory requirements related to this process and implement.

Qualifications

+ Associate’s Degree (AA) In nursing (ADN) Required

+ Bachelor’s Degree in Arts/Sciences (BA/BS) In nursing (BSN) Preferred

+ 1-3 years Nursing home facility experience within the last three years Required

+ 1-3 years Experience in the RAI process Preferred

+ 1-3 years Experience with Microsoft Suite (Word, Excel, PowerPoint, Teams, and Outlook) Preferred

+ Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.

+ Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.

+ Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation.

+ Being aware of others' reactions and understanding why they react as they do.

+ Actively looking for ways to help people.

+ Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.

+ Monitoring/Assessing performance of yourself, other individuals, or organizations to make improvements or take corrective action.

+ Adjusting actions in relation to others' actions.

+ Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.

+ The ability to see details at close range (within a few feet of the observer).

+ The ability to listen to and understand information and ideas presented through spoken words and sentences.

+ The ability to identify and understand the speech of another person.

+ The ability to exert maximum muscle force to lift, push, pull, or carry objects.

+ Registered Nurse Iowa Board of Nursing (IBON) (United States of America) Required

+ Basic Cardiac Life Support & First Aid - American Heart Association within 60 Days Required

+ Dependent Adult Abuse Mandatory Reporter - Iowa Department of Human Services within 180 Days Required
