Job Title: Patient Advocate

Hours Per Week: 40
Schedule: Day/Evening, Variable 8 hour shifts
Position Summary: The patient advocate will assist refugee patients in entering the Rochester General Medical Group and serve as an interpreter for the patient in both medical and non-medical appointments Key Responsibilities: •Track new refugee patients entering the Rochester General Medical Group. •Attend medical appointments with patients. •Make appointments on behalf of new refugee patients. •Arrange transportation if necessary. •Attend the appointment with the patient if necessary. •Arrange a follow-up appointment for the patient if necessary. •Provide interpretation for patients during medical appointments, social worker appointments and over the phone with RGMG staff if necessary. •Attend appointments outside of RGMG to assist patients with interpretation and understanding of instructions at that appointment. •Provide communication with RGMG physicians and medical staff about the health and healthcare related issues of refugee patients registered within our system. •Attend RGMG in-service and staff meetings at CFHC, GHS Pediatrics, and GHS Internal Medicine and speak with staff about cultural issues affecting refugees in our community. •Assist the Project Director in developing culturally appropriate training materials, signage, and patient education materials. Minimum Qualifications: •High School diploma or equivalent required, Associate’s degree or equivalent preferred •Previous health care experience preferred •Strong computer and communication skills, with previous experience using Microsoft Excel •Able to read, write, understand, and speak English comfortably •Familiar with either Burmese or Bhutanese culture and fluent in respective language Required Licensure/Certification Skills: •Valid Driver’s License

NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)
PHYSICAL REQUIREMENTS:
S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
