Job Requisition: REQ3586 Employment Type: Full-Time Shift: Rotating Hours Per Week: 40 hours
Hiring Range : $58.88-- $74.09
The actual compensation for this position will be determined based on experience and other factors permitted by law.
Responsibilities:
• Medication History Collection: Accurately collects and documents patients' medication histories upon admission to the ED.
• Medication Reconciliation: Performs medication reconciliation to identify and resolve discrepancies between patients' home medications and hospital orders.
• Collaboration: Works closely with physicians, nurses, and other healthcare professionals to ensure accurate and safe medication use.
• Patient Education: Provides education to patients and their families about their medications, including proper usage and potential side effects.
• Documentation: Maintains thorough and accurate documentation of all medication reconciliation activities.
• Quality Improvement: Participates in quality improvement initiatives to enhance medication safety and the reduction of errors.
• Training and Development: Provides training and support to other healthcare staff on medication reconciliation processes and best practices.
Minimum Qualifications
Education: Doctor of Pharmacy (Pharm.D.) degree required. · Experience: One year (1) clinical pharmacy experience in an emergency department or acute care setting preferred.
Certifications / Licensure: Pharmacist credentialed from the New York Board of Pharmacy obtained within six (6) months from hire date or job transfer date required. Board Certification in Emergency Medicine preferred.
