    CASE MANAGER

    We are excited to invite a passionate and experienced Case Manager to join our team! Homes for the Homeless (HFH) is searching for an experienced Case Managers for our City Sanctuary Facility for Families w/ Children located in Staten Island, NY. As the Case Manager, you will play a pivotal role in our organization, assisting families in providing them with the necessary care and resources needed.     Homes for the Homeless operates shelters for families experiencing homelessness in NYC. Our family residences are more than just a place to sleep. Families can make the most of their stay through accessing an array of onsite programs. Many of these programs are also available to people living in the local community. Our goal is to have a positive impact so families can thrive long after they leave our shelters.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    + Maintain and update digital and physical records of all families as required by governing agencies.

    + Collaborate with clients to develop service plans based on an assessment of each family’s needs.

    + Assist families with connecting to community resources, support groups, and cultural services to provide social support.

    + Assisting families with referrals to outside agencies for legal services, education, parenting, medical, and mental health services.

    + Maintain and update case files bi-weekly to document progress.

    + Adhere to all city and state guidelines for maintaining documents.

    + Conducts unit inspections.

    QUALIFICATIONS

    + Bachelor’s or Associate degree.

    + Fluency in languages other than English, particularly Spanish strongly preferred.

    + Knowledge of Outlook, Word, and Excel

    SALARY:

    $45,000 - $50,000
