    Community Health Center, Inc.     Job Description:

    + JOB SUMMARYThis Case Manager provides support and care coordination in the Mobile Crisis Program, working in collaboration with a multi-disciplinary team of psychiatric APRNs and master’s level clinicians to serve children, adolescents and their families seeking immediate CGC services.

    Roles and Responsibilities

    + Screen and triage calls from 211 and the community to dispatch to appropriate services

    + Conduct, and accompany clinicians on, evaluations of children and families in office or community and follow up in person or by phone as needed

    + Provide case management to crisis cases as directed by treating clinician

    + Collaborate with clinical staff in conducting emergency evaluations and developing and implementing safety plans

    + Work with families to develop and implement service plans, based on specific priorities identified by the family and treating clinician

    + Initiate creative problem-solving strategies to meet unique needs of clients

    + Help families, victims, and witnesses of crime and other potentially traumatic events, access needed services and supports, while fostering their development of knowledge and skills to advocate more independently with community agencies, schools, etc.

    + Develop and maintain positive collaborative relationships with community agencies and professionals to enhance resources to meet the needs of clients

    + Assist in preparation of agency reports, statistics and proposals

    + Assist Program Director in collecting billing and other required documentation as needed

    + Provide interpretation for clinical staff in sessions with families for whom English is not their preferred language

    + Actively participate in interdisciplinary treatment planning team meetings, supervision and other staff development activities

    + Utilize the agency’s electronic health record system to ensure proper and timely completion of clinical documentation, program statistics, and outcome data

    + Other related activities as assigned

    QUALIFICATIONS

    Required Skills and Education

    + BA / BSW Degree, preferably with a mental health related major

    + Minimum one year relevant experience, preferably in a child and family mental health setting

    + Familiarity with mental health diagnosis, treatment, and service systems a plus

    + Verbal and written fluency in English and bi-lingual fluency in Spanish

    + Experience and competence working with individuals and families from culturally diverse backgrounds

    + Computer skills, including MS Office, required; familiarity with electronic health record systems a plus

    PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

    + Hours will vary; our schedules range Monday-Friday from 6am-10pm and weekends.

    + Minimal physical effort. Must be able to operate computer and telephone continuously. Regular regional travel as necessary.
