    The clinical Pharmacist provides direct patient care that optimizes the use of medications and promotes health, wellness, disease prevention, and disease maintenance. The pharmacist also educates interdisciplinary teams on pharmacotherapy and develops processes and procedures to improve care.

    DUTIES AND RESPONSIBILITIES:

    + Participates in interdisciplinary rounds for Medical/Surgical patients and other patient care units.

    + Review medication profiles with patient problem lists to ensure complete disease-state pharmacotherapy treatment.

    + Adjusts medication dosage and frequency based on patient-specific pharmacokinetic/pharmacodynamic data.

    + Completes Medication Usage Evaluations (MUEs)

    + Assists in the review and development of medication order sets.

    + Participates in interdisciplinary committees.

    + Documents medication recommendations and monitoring plans in the medication record.

    + Co-precepts Introductory Pharmacy Practice Experience (IPPE)/Advanced Pharmacy Practice Experience (APPE) interns.

    + Prepares and dispenses all medication for hospital use according to pharmacy ethics, in conjunction with state and federal laws, rules, regulations, recognized standards, practice guidelines, and policies and procedures.

    + Reviews all medication orders for completeness.

    + Evaluates all medication orders for appropriate dosing for the condition and for safety, assessing metabolism, elimination, drug allergies, and drug interactions.

    + Provides dosing calculations and drug consultations.

    + Performs adverse drug reaction (ADR) surveillance and reporting.

    + Evaluates all medication orders for dosage, schedule, interactions, allergies, and compatibilities before dispensing medications.

    + EDUCATION AND RELATED EXPERIENCE :

    Required:

    + Bachelor of Science in Pharmacy Degree

    + Valid Massachusetts Pharmacist License

    + ASHP-accredited PGY1 pharmacy practice residency with at least 3 years of hospital pharmacist experience.

    + Maintains Continuing education credits as required for licensure by the Commonwealth of Massachusetts. Provided evidence of continuing education credits to the Operations Manager or designee.

    + Strong oral, written, and interpersonal communication skills.

    + Demonstrates leadership skills

    + Exercises initiative in process improvement and project management.

    Preferred:

    + Doctor of Pharmacy degree
