JOB SUMMARY
Provides advanced practice health care services as well as documentation of services, collaborates with colleagues and clinical director, supervises assigned support staff, participates in quality assurance activities, and demonstrates a commitment to quality health care.
EDUCATION & EXPERIENCE
Minimum Qualifications:
+ Completion of a graduate level (or higher) nurse practitioner program accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
+ No experience required
LICENSES, REGISTRATIONS OR CERTIFICATIONS
Required:
+ Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP)
+ Licensed Registered Nurse (RN) by the Texas Board of Nursing (TBON)
+ Licensed Advanced Practice Registered Nurse (APRN) in a nurse practitioner clinical area of specialization by the Texas Board of Nursing (TBON)
_Current RN’s who have not yet completed their nurse practitioner program or recent graduates who have not yet obtained their ANCC/AANP certification or APRN nurse practitioner license are eligible to apply for this position. However, official employment is contingent upon the successful completion of an accredited nurse practitioner program, ANCC/AANP certification and APRN nurse practitioner licensure._
Job Duties:
Performs responsible clinical care in collaboration with the Geriatric Medicine providers and under approved protocols, manages the medical aspects of care in the observation, assessment, diagnosis, intervention, evaluation, rehabilitation, care and counsel, and health teachings of persons who are ill, injured or infirm or experiencing changes in normal health processes; and in the promotion and maintenance of health of prevention of illness in a clinical setting.
+ Provides primary care for patients in the geriatric house call program, as well as residents living in both independent and assisted living settings.
+ Performs individualized history and physical examinations for diverse populations inclusive of social determinants of health.
+ Orders and interprets diagnostic testing in accordance with relevant health concerns of the patient.
+ Obtain laboratory testing; includes blood specimen draws, catheterization, flu swabs.
+ Training for joint injections.
+ Precepting and mentoring school of nursing and school of medicine students.
+ Diagnoses acute, episodic, and chronic diseases.
+ Develops a comprehensive plan of care that encompasses evidence-based practice guidelines, practice protocols, Medicare, Medicaid, and other state and federal regulations.
+ Prescribes appropriate pharmacological and non-pharmacological treatments.
+ Provides appropriate education for current health diagnoses as well as anticipatory guidance for health promotion and disease prevention applicable to the individual’s health and socioeconomic status.
+ Monitors plan of care for cost-effectiveness and the achievement of expected improvements, while openly seeking feedback from patients, significant others, colleagues, allied health, appropriate referrals, and collaborating physician to enhance quality outcomes.
+ Works independently to full scope of practice in compliance with the Texas Nurse Practice Act with acknowledgement of collaborating physician availability for consultation and collaboration.
Salary Range:
Commensurate with experience.
