    GA Family Services - Licensed Practical Nurse (LPN)/Discharge Planner Reports To: RN Supervisor

     

    Job Type: This full-time position typically requires 8-hour shifts, Monday through Friday. Occasional flexibility with hours may be required.

     

    Salary Range: $19.50 to $26.21 per hour, commensurate with experience

     

    Purpose: The LPN/Discharge Planner is responsible for the coordination of medical and discharge planning services for our therapeutic foster care youth.

    Qualifications:

    Applicant must have graduated from an accredited school of professional nursing, hold a current NYS LPN licensure and CPR certification. Past and successful experience as an LPN is preferred. Must have a valid drivers' license. CPR and First Aide Trainer is also preferred.

     

    Please note: Although this position is based out of our West Seneca location, occasional/as needed travel to our Jamestown office is required.

    Job Description:

    + Manages, tracks and ensures all medical appointments for all youth are scheduled in time frames established by state regulations

    + Manages the medical case file for all assigned foster care youth

    + Be available to staff for consultation regarding medical concerns

    + Supervises foster parents administration of medications and completion of medical forms

    + Ensures foster parents are trained and educated in regards to medication administration, medication logs and side effects

    + Authorize billing for all medical services and seeks payment authorizations with MMCPs within the 29-I regulations

    + Completes discharge planning in accordance with all 29-I regulations, including coordination with schools, community providers, and maintaining documentation to allow for HIPAA appropriate communication throughout the treatment and discharge planning

    + Provide basic first aid/health care services to youth as appropriate, as required by 29-I regulations

    + Coordinates with the Medicaid Managed Care Liaison (MMCL) and RN Supervisor for all medical needs for youth

    + Completes 24 hour initial health assessments of youth entering foster care settings and moving foster homes, as needed

    + Monitor and enter Connections health information for all clients in TFC program

    + Assist with training of staff and foster parents including first aide, CPR and universal precautions, psychotropic medications and administration of medications

    + Maintain communication with Clinical Supervisor on all Mental health needs and appointments for youth

    + Updates medical record tracking, and assures that client medical records and appointments are tracked with appropriate diagnoses in GA systems

    GAFS Organizational Responsibilities:

    + Engages in regular supervision with own supervisor, including scheduled evaluations

    + Actively participates in all in-service programs and scheduled meetings during your on-days

    + Maintains ongoing and open communication with team members and promotes relationships of trust and respect with residents and staff

    + Fulfills obligations inherent in the major systems of the GAFS and the LSS Human Resources Policies

    + Demonstrates a commitment to organizational and department goals

    + Shares positive and negative information directly with team members and confronts others; to utilize the organizational chain of command

    + Practices good listening skills and communicates well both in writing and verbally

    + Demonstrates personal and professional ethics, attitudes and value systems which enhance team functioning and to abide by the Standards of Confidentiality

    + Upholds child and family rights as outlined in State Office of Child and Family Services mandates and GAFS procedures
